      I obtained an appointment for a tooth cleaning and I was quoted $135 for the service. I accepted the appointment. After the appointment I was told that I owed $315. When I questioned this, I was told that they had done what they called a "debridement". The problem is, I was never consented for that service, never told I was getting it, nothing until time to pay. I objected to the fee and the dentist decided that I could pay the $135 that I had agreed to. However, I had to sign a record release because I was told to never come back to the practice.
