    We were initially told at date of service that our insurance would cover the full procedure. We received a bill for $248.74 on 11/27/24. At the time, I talked to ******** and she said the bill was paid in full and we were done. Today, on 6/23/25 I received another bill for $97.26. The statement is not showing the $248.74 paid on 11/27/24. When I spoke to ******* today, she was said it was a problem with my insurance and not on Keystone Dental Mini Implant. She could not explain why the $248.74 I paid was listed on the statement.

    patient brought a panorex from their dental office.   Patient was charged for another one at $97.26 It was written off..  Patients insurace made another payment and the $97.26 was again just written off since this letter brought it to my attention.  Patient's balance is 0

    In 2020 during the Covid outbreak I gave Keystone Mini Implants a check for $6202.40 to have my top front two teeth replaced and another tooth on top to the right was to have an implant cover. There was a broken tooth in this location to the right and Dr ****** extracted the tooth and replaced it with an implant on the same visit. He also performed work on my front teeth. When I returned for my next visit, the implant on the right tooth was loose so Dr ****** removed it. He then went to work on my front teeth. While on the front teeth he performed a procedure which was extremely painful and "I'm sorry but I had to do that". Fast forward through the Covid years to 2024. I received a check from Keystone Mini Implants for in the range of $30. I wrote a letter to Keystone explaining that the tooth on the right was never completed.I received a phone call from Keystone Mini Implants on 5-23-24 (which you can see from my phone logs) and spoke with *******. I explained the situation and she spoke with the doctor and he wanted to know if I wanted him to finish the implant to which I said no. ******* said that they owed me over $2,000 dollars and it would take a month for payment. I Called back on 10-1-24 and I was told that there is no record of me calling or my letter in my file. As you can see by my phone log there are calls made. All that I am asking for is a refund for work not done. I am sorry that I do not understand dental jargon so I sent a copy of my insurance record to help identify the tooth in question.Thank you for your time.

    Called patient upon letter with check that was mailed to him dated 01/30/2024 in the amount of $31.40. He never returned offices call until recently. We are refunding him for what he paid on tooth #5 as it was not completed.  When we talked to the patient he did not want to have the work done.  no issue doing a refund on our side. Refund completed in the amount of $2400 today and will be mailed to patient once proper approval has been met.
