    In January 2023 I was charged $309 by ***** *********** for a pre-surgery lab tests ordered by Dr. *****'s office of Penn Medicine on 9/14/22. Five of the tests were denied by ******** as "medically unnecessary". ******** told me they did not get the necessary documentation from the provider to demonstrate the medical necessity. ***** had reached out to Penn several times for this documentation. Since I learned of this, I have spent countless hours and over 40 phone calls to ********, ***** and mostly to Dr. *****'s office in an effort to have them send the necessary documentation to *****/********. I was told several times by Dr. *****'s staff that everything was fixed and I only had to wait 30 days for ******** approval, only to find out, later, that nothing had been accomplished. This caused further delays. I wrote to Dr ***** on 5/16/23 and met with him on 5/19/23. These efforts accomplished nothing in terms of getting ******** what they needed. I spoke to **** *****, Patient & Guest Relations who never got back to me, in spite of several follow up phone calls. On 7/19/23 I wrote to ***** *****, COO Department of Orthopedic Surgery with no response. At this point, I am out $309 because Dr. *****'s practice has been totally unprofessional in their refusal to address what I understand to be simple coding problem. I also believe that several of my Patient Rights are being violated by this facility. I was also told by the Orthopedic Surgery Practice Manager that he would waive my co-pays and parking for all follow up appointments associated with my surgery due to the inconvenience this issue caused me. This has not been done this. I filed an appeal with ******** on July 12, 2023 but it is considerably beyond their deadline for filing. I learned on 7/5/23 that ***** archived their file and can no longer be of help to us.
