    Dr Magda office put a crown on my sons tooth in May 2023. On June 11, 2023 the crown came out. I brought the piece of metal to the office on the following Tuesday and it was confirmed to indeed be the crown that was just put in. They scheduled an appointment for him to replace the crown. Dr Magda stated there would be a charge for this and it was not covered by insurance because it would be considered a second crown. I told her i would not pay for her to fix the problem and she should stand by her work and replace it at no charge. She told me and my son to leave and that I can not talk to her like that. I am seeking reimbursement for the original charge as her work was not done with any quality and she refused to make it correct.

    To Whom It May Concern, 
    Please be advised that this office, including all clerical and clinical staff members strive to provide the "utmost" in dental care to all of our patients. 
    When an accident has occurred to a patient outside of this office in their (patients) day to day lives to damage work rendered by a dentist, this in no way the direct result of the work completed by the dentist/office, Therefore it becomes the responsibility of the patient and/or guardian to cover any additional costs that may be incurred to 
    correct the issue. 
    Synopsis of incident: 
    Patient's father came to our office on June 13, 2023 and stated that his son's stainless steel crown was out and requested an appointment to see the dentist. We scheduled the patient to see the dentist for 6/15/23 @ 4:00PM. 
    Patient and father arrived in our office on 6/15/2023. Patient's father advised our front desk staff that the crown "just fell out". Upon the seating of the patient in our clinical treatment area by our clinical staff, the patient advised us that "the crown fell out while playing basketball when the ball hit his cheek" 
    Attached please find our clinical notes, for your review, of the incident made at the time services were rendered, by all parties that had direct interaction with the patient and his father. The clinical notes give a detailed explanation of the events that occurred, to support the synopsis noted above of the events. 
    Please be advised that the patient's insurance policy does limit how often a procedure can be performed, as set forth at the discretion of the employer providing benefits.
