      The first date of service was 3/5/2024, the charge was $143.00. Insurance company paid $105.46 in full on 5/8/2024.
      The second date of service was4/5/2024, the charge was $140.00. Insurance company processed claim on 5/1/2024, with $61.12 contractual adjustment and $78.88 applied to deductible.
      When I First received the bill, I contact the Pathology Consultants INC, unfortunately they outsourced their billing services oversee, after I provided all the payment information provided by my insurance and suggested that the actually service location, * ******** ******* **, might have received the payment, the oversee representative said "we cannot legally call your insurance, you will need to mail us the information." I requested to speak to Supervisor because there is no SUCH law in the US to preventing provider from contact insurance companies as I signed HIPAA to consent my provide communicating with my insurance. Either the oversee company the Pathology group outsourced to did not understand US HIPAA law, or the oversee company illegally handling our health care information without signing proper HIPAA consent. The Supervisor said he would pass on the information to their billing team. In the meantime, I mailed the EOB, also filed complaint on their website (with no response). On 8/9/2024, I received threatening letter from Pathology Consultants INC in RED paper and BOLD letter stating they will be sending me to collection for DOS 3/5 and second notice for DOS4/5. For 20 years, I have never had collection, they are ruining my good credit all because they have company with incompetent oversee staff does not understand US HIPAA law, and possible illegally handling our health insurance claims.

      I can see that the carrier listed is ***** ***** ******, which is not a direct carrier, but rather a third party repricing agent.  *** did not receive an EOB from the carrier.  The adjustment was applied to the 04/05 DOS on 08/22, and the patient paid the $78.88 on 09/14.  The adjustment was applied to the account for 03/05 DOS, but it does not show the payment in the amount of $105.46.  The account was updated to ** **, for payment research.  It was previously placed on 2 separate 30 day holds, until the EOB copy was received.

       

      I did locate a payment that is posted to the unapplied cash account, as it was received without an EOB.  I have entered a request to have the payment transferred to the patient account.  I have asked our Call Center to reach out to the patient.

      The Call Center has reached out, but did not reach the patient.
