    (I gave you my roommates number because I don’t answer unknown but we’re always together) I used to have a therapist name Pam A******* at Table rock integrative health and wellness practice She is no longer seeing as many patients so I found a new therapist. During my time with Pam, she told me that my copay was $20 and I saw her every 2 weeks and I paid $20 each time, equaling $960 by counting from June 1 2019 until august of 2021 at $40 a month (40 being the total I paid a month). I am actually the one that found their mistake by reading my ******** notice sent to me every month and every one of the parts where it shows what you’re supposed to be paying as a copay and my copay that whole time was a zero copay yet I was being charged $40 a month. I immediately called Sandy, the employee of the practice that handles all of the book keeping including insurance and she looked it up and found that, yes indeed, you have no copay which means I’m owed a refund of my money in the amount of $960 ($20 every two weeks so 40 total in the month from June 2019 to august off 2021.) I have asked Sandy repeatedly and I mean repeatedly about my refund. Usually she will tell me that she will refund me as soon as she was done with whatever she was doing but last time I asked her where my refund was she was having “computer problems” and said she would get back with me as soon as the computer is fixed and that she’d cut the refund check, and that of course is not what happened. Since then, I have been completely ignored and I imagine they just don’t want to pay the $960 because of the amount owed to me. They cannot refuse to pay me back for what they did and the errors that happened between my old therapist Pam and Sandy, this is fraud, this is a deceit. Sandy is Dr. D********’s only employee at this table rock business. I just want my money back since they know they owe it to me, that’s all I want. I also would like for Sandy to cut my refund and allow me to pick it up.

    I  am going to address the issue of our patient's complaint in two stages as I will explain below.


    First our patient herself is not fully knowledgeable about the ****************** ******** Beneficiary program (hereafter referred to as ***) as she insisted she was not on ********, although it is listed as a ******** program. nor did she know when she became a *** patient.   When she first brought this issue to our attention, we went online and researched it.  There is a code that appears on the bottom of the ******** statement - ****, that also appears next to the patients to whom it applies.  I am sending to you separately  a picture of the first claim when this code was applied - 8/17/2020, where the **** is listed and is defined at the bottom of the page.  From then on, it is listed on all her claims.  We stopped collecting copays once we were aware of this after 1/25/2021.  Furthermore, on a handwritten sheet which I am also scanning and sending to you, I list the claims where we refunded to her what I show was applied to these claims.  (From 8/17/20 to 12/17/20 she met with our therapist Pam A*******, and from 12/23/20 to the present she is meeting with our therapist John E***.)  I gave her a copy of this sheet when she met with me at our office last month.
    The 2nd issue is the amount paid at each appointment which was definitely not $20.00 every time.  I am still researching this but most likely the amount of the copay due was deducted from the 20.00 and the balance applied to other appts and/or no show fees.
