      On November 27th ******* ******( I believe is a nurse for the facility) on December 4th ******* **** psychiatrist from this facility. (********* ***** patience and ******* ******* is my primary that is aware of this situation and the license pharmacist from ****** Rockwall provide the information of the person who prescribed the medication on December 27th)On November 27th ******* ****** (who I thought was ******* **** the person that I was supposed to see on my first visit, diagnosed me suspecting ****************. From November 27- December 3 taking medication: -Queriapine Fumarate 50mg at night -queriapine Fumarate 100mg after 2 weeks -********************** 2mg at night -*********** 100mg half table twice a day First visit of the person who was pretending to be doctor **** (facility never clarify was someone else) November 27th. Incident happened December 2th 1:45am in my bathroom Ive had palpitations and dizziness I blackout and fell hit my head and short loss of memory, during the same day at 8:45am I call Pionner healthcare clinic they take me right away to check on me because I was having delusions and sort of cognitive impairment so they order me to take a CT scan or go to the ** same day.Was not diagnosed or history of medication of this matter. Only condition is sleep apnea, PTSD, OCD, anxiety and depression from therapist ***** ******, he mentioned that I have autism neurodivergent triggered by my trauma.Previously was treated by **** ****** therapist over a year. I went to this facility for recommendation of *************** (resources for living ) to treatment for my PTSD. There is not discharge ******* **** on December 4 she prescribed ********** 5mg. (I havent pickup from the pharmacy) I reported the incident with the supervisor and management that havent reached out to me yet. I have called them several time. Im the one who have been trying to get in contact with the facility management

      Ive tried several times to contact the management of this facility without success. I would like to get some referrals so that way I can get better assistance specifically in the legal matter for neglecting the patients
