    The date of service was 6/20/2022 in ***************** **. Radiology services were provided after a bad car accident. First, I was put into collections before a bill was received. I mailed insurance info when I finally did get a bill both over phone and in writing on multiple occasions. I have followed up and have names and dates of each call and conference call woth my health insurance provider. To date, they have filed no claim woth my insurance after 1 1/2 years. Their company shows complete lack of accountability and follow through as well as incompetence and failure to escalate and follow through with management on their billing number. I filed a complaint also with **************** insurance against insurance billers as well over this. I would like the claim actually filed and processed then an accurate bill sent after the claim is processed so this can be handled and I can move on with my life.

    I was billed by RAF of Tampa. one a Lung Scan and two a Thorax Scan. The Thorax scan was never ordered by my Physician. #1 bill is $772 and #2 bill is $575. I would never of had this done if I knew it was that much money! I was told by the scheduler that because I was 78 years old medicare would not pay but the bill would be between $50 to $100! I called RAF of Tampa and was given a name of a person to contact, Janet at 813 251 5822 ext 1619. I have placed four calls to Janet and gave her my account number and asked. her to please return my call, to include asking her to return my call with another person I could talk to if she wasn't the correct person. No call Back!! I again called RAF of Tampa and after being on hold for a long time was hung up on. I also have called the number in Dallas which was on the bill if you had questions. Unfortunately English was not that persons first language. She was very difficult to understand and I kept having to ask her to repeat herself. Finally she said I should call back in 2-3 weeks to see if this was resolved. I am on a fixed income and there is no way I will be able to pay RAF of Tampa $1347 for reading these Cat Scans. Especially as one exam was not even ordered and I was told the other one would be $50 to $100. I am asking to just discuss this bill with someone! I do not believe this to be an unreasonable request. Furthermore I sent 43 years in the medical field prior to retirement and this is unacceptable treatment. I am ***** ******** at XXX XXX XXXX. Please solve this serious problem I am having. Thank you so much and hoping that you will be able to help me.
