    My therapist and admins of alta loma transitional services were specifically told not to release any medical information to any person due to stalking and **** , myself the victim. Multiple phone calls to and from **** *. And ***** , to my ex husband regarding specific info about my medication , program medical participation , and other hippa protected data was disclosed by exhusband text screenshots of admission to gaining sensitive and protected information. And the retaliatory treatment after filing a proper grievance to policy writen and federal law of client/paitents bill of rights , still not posted publicly or in a conspicuous place accesseable to and community patients and potential clients and/or members of the public.

    These are screenshots of conversation with my ex , whom has knowledge of HIPPA protected information from ALTA LOMA TRANSFORMATIONAL SERVICES *** ..

    ALTA LOMA is a health services provider dealing with inpatient mental health provision and case worker services with outside medical providers on an appointment only and out patient services status.

    The information leaked in violation of HIPPA is that of their therapist and inhouse psychologist Dr.****.

    Services provided cost roughly $1000 per day , i self admitted on September 3 2024 and was forced to leave on October 10 2024 after retalitiry medication was prescribed for filing a fact based grievance, accoringly to the ************** policies.

    Grievance remains un-resolved in regaurd to patients rights and violation of rights and texas and federal law required posting of patients rights in a conspicuous and publicly accessable place.

    To Whom it may concern,

    I can neither confirm nor deny that anyone by the name of ******* ********** was in our care.  For me to comment directly on any person receiving services from *********, a signed Release of Information would need to be in place, even for an entity such as the Better Business Bureau.


    Alta Loma specifically treats individuals with significant mental health disorders.  If someone were to leave early, we are required to notify their emergency contact. Alta Loma also collects the fees for treatment services upon admission and as such, if someone were to prematurely leave treatment, refunds are promptly processed on a per diem basis back to the Guarantor on the signed financial agreement.  The Guarantor and the client being served are most often not the same individual, due to the fact that the individuals we serve are not financially capable of paying for the services rendered by Alta Loma, most often as a result of their diminished mental state.

    I am rejecting this response because:

    Does not address the original complaint of HIPPA violation by disclosure of privileged health information and findings of doctors and therapists ; including medications , diagnosis(s) , and treatment planning. No release of information was ever signed in regards to any individual or emergency contact during or after services , except for the Better Business Bureau for investigation.

    Text messages from a non authorized party specifically naming Alta Loma employees in violation , diagnosises, medications and unauthorized communication in violation of company policy, state and federal HIPPA laws and protections provided to BBB and other agencies stand as proof undeniable of violation of patients rights , and violation of  federal laws. 

    Again the late response of Alta Loma side steps the complaint never addressing the actual complaint through deceptive distraction to irrelevant matters to avoid a direct explanation, admission , or denial of the allegation(s) and fact evidence of leaked protected information provided of third party messaging and now individual willing to provide statements of fact HIPPA violation given without authorization both during services and post services ex-parte communication with non authorized, non provider , non legal representative by therapist and administrators of Alta Loma Transitional Services.

    The deception and non protection of clients and clients info and HIPPA  continue into liability and non accountability by a criminally negligent corporate entity trying to hide behind the protections they violated and never provided per company policy and federally required protected health information covered by HIPPA laws and paitents rights.

    The response from the complainant is incorrect.  There must be at a minimum an Emergency Contact release of information signed upon admission to any of our programs.  The person with whom the text messages attached is the individual with whom we had an emergency contact release.

    Furthermore, a refund for the services rendered has already been processed to the guarantor with which we had an agreement. This is also the individual in the text messages with Mr. *********************** was no HIPAA violation as the communication in these text messages is between the guarantor and the complainant.  I understand that Mr. ********** feels as though his rights were violated but we did our duty of notifying the individual for whom he gave us permission to contact in case of an emergency, which would also include us discharging someone from our program. We must give a reason for the discharge.

    The client Rights that Mr. ********** is saying are not posted is also incorrect.  These are posted at our licensed and accredited clinical buildings. Mr. ********** was discharged from one of our group homes which is not licensed or accredited through any higher organization as the license for these does not exist in the state of ******
