    I paid $160 in advance for the biopsy of an oral cyst on Nov 6, 2024. Since then I received an insurance statement showing $134 as the amount billed by the provider to my insurance, and an insurance payment of $22.91. I calculate that I should be refunded $26 for the difference in the pre-paid and billed amount, and $22.91 for the amount my insurance paid, for a total refund of $48.91. The billing confirmation lists tel# ************ or email SOD-*************** for billing questions. I have tried very hard multiple times to contact the provider by telephone without success. I have emailed 3 times with details. So far I have gotten no response whatsoever.

    I am writing to file a formal complaint against the UT ************* in Houston regarding my orthodontic treatment from a few years ago. After spending over $3,000 on braces and permanent retainers, I have faced ongoing issues that have yet to be resolved.Since the initial installation, Ive had to return to the clinic more than five times to repair the retainers, yet the problems persist. The retainer on the upper part of my mouth has broken again, and Ive noticed widening gaps between my teeth. After consulting three independent orthodontists, I was informed that the retainers were incorrectly installed on the wrong part of my teeth, which has led to repeated breakage and shifting of my teeth back to their original spacing.I attempted to resolve this matter directly with the UT ************* by emailing and calling the office multiple times. However, I was either hung up on or received no response from Office Managers ******* and ****. Their lack of communication has left me frustrated and without a solution.Given the significant cost and the continued issues caused by this improper installation, I am formally requesting a full refund for my treatment. The improper care has resulted in my need for further orthodontic work, which should have been unnecessary.

    I became a patient of UT Health Houston School of Dentistry on June 6, 2023. On the first appointment I was told that my teeth couldn't be saved and that I would either need to use dentures or that I had to get implants. No other options were ever given. So I opted for implants without ever given full information on what this would mean for me health wise. On Jun 27, ************************************************************************************************************* a panic attack. The supervisor to the student dentist didn't bother to offer to call the ambulance or felt the need to let anyone from my family members know what had happened. I could bearly speak because of the extractions. Instead they stopped the procedure, closed me up and sent me on my way home. I had to visit the er that day and get xrays. Now I have a huge bill hanging over my head and it's affecting my credit score. Something that was brought on because of the pain I was experiencing and the anesthesia I had. However until today not one supervisor had ever asked If I was ever ok after that scary episode. I found a dentist who's taking care of me now. He says he can't believe they were going to let me stay without any type of immediate dentures for so long that them leaving me like that had caused my smile to collapse. Now dentures don't fit properly and I'm still waiting to get my smile fixed. On top of all of it I have been waiting to get a refund since Sept 22 ******************************************************************************************************** on several occasions I would get a refund for what I paid upfront and haven't received anything yet.
