    I have been dealing with ***** and the social worker at US Renal Care on Heritage Trace Pkwy in **********, ** trying to get a form 2728 sent to three hospitals to be able to put on transplant lists. None have received. ***** has been making excuses such as Labcorp wouldnt talk to her for lab results and she cant upload documentation. This has been going on for weeks. This facility is endangering my life by prolonging unnecessarily this form being sent and stopping me from being put on a transplant list. I finally provided labs since she couldnt figure it out. She stated she would provide to social worker who would complete and send to hospitals. Nothing has been done. ***** is now making excuses to the hospitals shes having trouble uploading information even though she advised us the social worker would be completing. I received a call from Krissy at *********************** 11/1 saying she had just spoke to ***** and ***** states she is having trouble uploading. We call the facility and told ***** isnt in. Krissy did ask for a manager and was told one wasnt available. Again, this has been going on for weeks and getting no help from this facility. I believe ***** is deliberately making excuses. She also said the social worker completes to us but she told Krissy she completes. Either way, nothing is getting done and she is stopping me from going on transplant lists at ************ ******, ************ ********** and *****************. ***** and US Renal Care are jeopardizing my health. I feel deliberately. I changed facilities as I felt I wasnt getting proper care there. Possibly this is retaliation. I can think of no other reason why ***** is making excuses and not doing her job. When we spoke with her last week and dropped off copies of my labs, she said that would be the first thing she did. She said shed also gone on a cruise. So while shes having a good time not doing her job, shes prolonging my wait to get on a transplant list.
