    My daughter was scheduled a 8:30am appointment at this location, the manager provided paperwork to fill out and asked for driver license and insurance card, i provided both i sat with daughter to work on filling out paperwork together in an effort to demonstrate her how to fill out documents, the manager in a grumpy state stated that she needed to walk to the back with her while i filled out paperwork i stated to wait shortly while i finished filling out the paperwork, while in exam room i asked as to why she was taking excessively photo images without prompting her any instruction to wait for eye i could see my daughter just constantly blinking there was no correct posture instruction or eye placement in photo priorly the manager took offense to me asking questions about my daughters eye photos and said i was not permitting her to do her job, then i prompted to video record she stated there was a policy to record so she demonstrates there was one on the wall and the video recording stopped but audio recording continued while in exam room as phone was in my pocket, the manager then said she refused services because i was being problematic and brought ************** of which she was not the requested doctor for appointment and that the doctor would not be present for her exam then said that she continued to refuse services and the doctor was on her side because she was not having it with me i asked if i was not permitted to ask questions about my daughters exam she said yes but that i was just being problematic and that she refused services and that i could reach the corporate office for any other questions
