    We contracted with Form Pools and Spas W/ *************** for a complete landscape project bid out at over $450,000. We gave Form $147,000 for the landscape and $25,000 deposit for the Pool February 2022. Form notified us March 2023 that due to high cost increases, they could not finish our project based on the initial bid, but that if we agreed to pay the higher material costs, they could proceed. We agreed to everything, which increased our bids by about $50,000. Three more months of stalling, lies and ************* and early June 2023 they cancelled our project keeping all of the deposit money we had paid, Including an additional $27,000 that we paid for the plaster, but which was never bought or installed on our pool. Form pools stole over $200,000 from our family. They damaged a $120,000 engineered wall and left our home with dug up pits and mounds of rocks and dirt. This is the fourth bankruptcy for Chip the *****. They abandoned over ********************************* *********** away from Chip or ***, they are dishonest to the core. Hopefully, someone reading my review will be spared the same pain we went through.

    Form Pools and Spas is the worst company my husband and I have ever dealt with. They owe us well over $100,000 but have shown no intention to repay us. They basically strung my husband and I along for over 18 months, took over $100,000 of our money and didnt provide their goods and services. They had always been terrible at communicating, usually taking weeks to get back to us, but when all communication stopped in June 2023, after more broken promises, we got our legal team involved. After ignoring our legal teams requests, our team finally received an email which stated: Form is no longer in business. Have a nice day. The snarky response to have a nice day after knowingly taking and keeping our $100,000 + showed no remorse whatsoever. Looking back it seems Form tried every possible way to get more money from us without doing the work. They claimed decking prices went up since our contract and asked for $10,000 more, bringing just the decking price up to $45,000 for the lowest quality Trex decking. My new contractor is charging us $25,000 for the highest quality decking for the same project! We wonder what Form did with our money? Did it go to pay for someone elses project? Did it go to pay Forms bills? Whatever they did with our $100,000 +, apparently the owner and his team didnt have a problem taking our money and keeping it for their own purposes. Its hard to imagine such heartless people who must have thought they deserved our money more than us. We are devastated. That $100,000 + was a lot of money to lose.

    Form Pools is now out of business after abandoning multiple jobs and ripping off several customers including myself. Be advised that the same people that ran Form Pools are now operating as MTN MOD LLC under a different contractors license. MTN MOD LLC's website shows projects that were previously shown as Form ************* and their Houzz.com posting shows a glowing review for a completed project that wouldn't be possible since the contractor license is only current as of 08/01/23 and the company just started this Summer.

    I was informed by ************************* that his company - Form Pools LLC - would be abandoning my pool and spa project, despite paying all milestone payments on time and in full on my project.The *************************************** commenced action to revoke Form's license to operate today (4 January 2023) due to THREE reports of job abandonment in recent weeks.His reason for abandoning our project is that they aren't making money on it - he says it is due to material costs increasing over the past year due to inflation. They had to dig our pool a second time because they originally dug in the wrong location (they didn't read their own plans) - and had to dig it a third time because it sat for months and caved in. The tile job had to be ripped out and redone because the first attempt was done by an employee that was a landscape laborer and didn't know how to tile (the pictures would be laughable if it wasn't my pool). Reality is that they are losing money on my project because of having to do many expensive parts of the project twice (or more).It has been a miserable experience to work with Form and Chip. They do not return calls and the project has experienced many many delays due to poor project management and poor/shoddy workmanship by their subcontractors. I sincerely hope that nobody is put in the position that we have - no plaster, no pool or spa covers, broken equipment, incomplete plumbing install, coping that is spalling due to poor install quality of the deck jets, no warranty - and I have to hire another company to complete the job.I want others to be informed before making the mistake I did in choosing Form to build my pool and spa.
