      I am not liable for this debt with Uintah Basin Medical Center. I have never been to this medical center. I do not have a contact with Frontier Recovery Service. They did not provide me with the original contract as requested.

      I was contracted with a temporary staffing company in February of 2022 to work for an animal rescue organization as a week-long interview employee. While there, I needed to go to the local hospital to receive treatment for a cat bite. I took what I assumed was the correct insurance information from the animal rescue organization, received treatment, and left.

      I later received an invoice from the hospital, which I contacted the animal rescue organization and the staffing company about via telephone and email in August 2022. I was told via phone that same month that the issue had been taken resolved, and that the staffing company had taken care of the bill--I'm assuming with their workers' insurance.

      However, this month, I received notification by Experian that that debt from the hospital had gone into collections in November 2023. I was never notified by either the hospital or Frontier Recovery Systems about this debt.

      This is not my debt, and I was never notified about it's existence. I need you to remove this and certify to Experian that this is an error
