    On or about 21 January 2025 at or around 1:25pm I went to Ageless Men's Clinic Located at 1920 W 250 N St, Ste 1, Ogden, UT 84404, United States in order to see if I qualified for Testosterone Replacement Therapy. The front office informed me that it would be $125.00 up front however it would cover the first treatment as well if the patient qualified. I agreed to the blood sample in order to see if I qualified, pending my lab results the clinic called and stated that I was not a candidate. I write to inform you today that I believe sometimes a patient's lab results need to be tested twice so I am requesting that I receive another lab test or blood draw at no cost. I did not receive any treatment from this clinic that is located in Ogden, Utah. What I am asking for is another test at no cost to me to see if I am a potential candidate for hormone replacement therapy or not.

    21 January 2025 at approximately 1325 (1:25pm) I went to Ageless Men's Clinic located at **** * *** * *** *** ** ****** ** *****, United States to inquire about treatment for male hormone replacement therapy. I wanted to know if I qualify for the Testosterone replacement therapy and in order to see they stated I would need to have my blood drawn. The cost was $125.00 for the blood to be drawn however; I was informed that it would come with the first treatment. I went through with the blood draw and was told I would receive the first treatment. I received a call with my results but I was told that I do not qualify for treatment. I believe that I should at least get another sample of blood at no cost in order to get an accurate test. I believe that sometimes blood tests can be faulty so the patient would need at least one more sample before being denied treatment.
