    Date of visit: 1/11/2018 Amount paid after health insurance $ ******** Eye exam performed to determine whether my eyes need treatment. No treatment performed yet, and they billed me twice of $54.00 each, and amounts of $340.00 and $552.00 without any service associated with the charge. I came and the front desk promised to correct the bill. Not hearing anything from them for three years, then, I am in colection for the bill. Their lawyer called my work, trying to get me fired!

    Dear patient,


    We understand your frustration at this situation. After careful review of our documentation, we found that we had attempted to obtain your current insurance information multiple times, whether it was from your new patient documents from your referring provider, or directly from you. We reached out to Premera and ************** Services multiple times to obtain payment for your services, but Premera could not identify you as insured, and ************** Services stated that you were under another insurance at that time. 


    We also mailed 5 different statements to you, over the course of 3 and a half years, to which we did not receive any calls to our billing department to work out your bill. You stated that you came to our office and spoke with our receptionist, who had "promised to fix your ****** We have no objective documentation of this. We reviewed your chart, and your billing statement is accurate for the services and care rendered on 1/11/2018. 


    We are willing to work with the collection agency in regard to a payment plan. Please advise what would be feasible to you.

    I am rejecting this response because:

    The bill show that my health insurance already paid about $350, so your billing to ********** is the correct insurance, but the charges was for ~ $1500 for an initial exam. I expect any service costing above $300 be submitted to the insurance first, and inform me of the charge and let me decide if I want to go forward with it. You went ahead with expensive service taking pictures of my eyes for over a $1000, where my recent eye doctor did the same service for $25, and was paid by my insurance.
    The bill also has 2 item that weren't explained what they are for.
