      TL;DR breach of duty of care owed to pediatric ************************ against a disabled patient restricting / delaying access to medically necessary formula; expectation was set for a bridge for patient medical grade formula and has yet to be filled even after required in person visit was done; despite parent frequent calls and emails for status updates to medical facility. Starting from 03/14/2025 to current.04/14/2025 Call to Aveanna (spoke with ******) revealed that Sea Mar East Vancouver sent over medical forms that were not signed by a physician and not dated change request form. I was updated that Sea Mar needs to also send a form back to Aveanna called Certificate of ******* Necessity which is a normal form for medical grade formula. Email sent to ***************** communicating what Aveanna needs to move forward. Call to Sea Mar, spoke with ****** where ***** advised that she put an asap request to have a supervisor call me back, put a note in both childrens profiles and said to call back tomorrow if no call back is made to me today. Advised to ******* that the following will be done by end of business day today:I will be filing the following complaints for each child: breach of duty of care owed to pediatric ************************ against a disabled patient. These complaints will be filed for each child against ********* nursing license as well as Attorney General and ******************** complaints against this specific Sea Mar facility for each ******** family is out of pocket $1066.84 formula expenses to date and adding more as time progresses. Nursing license complaints have been filed as of ******* at ****** PST.

      I have been having some issues with my health that needs to be tended to and I decided to go see my health care provider Sea Mar community health centers in *******. See my arm has a walk-in clinic from 7:00 a.m. to 8:00 p.m. I decided to go see my provider on Wednesday August 21st 2024. I made you a clinic about 7:30 and went inside to sign in about 7:40 and the receptionist told me that the doctor was not seeing any more patients for today. I feel that if your hours are from 7:00 to 8:00 p.m. then patient should be seen until 8:00 p.m. I feel that they did not do their job as healthcare officials being that they don't know the nature of my illness only to be turned away and told me that the doctors not seen anybody else today. I didn't say it to the clerk that I didn't agree with them and that I should be able to see the doctor to get my check up because of the health issues I was having. The receptionist continue to tell me that she was sorry but I need to come back tomorrow and I told her that I can't come back tomorrow I need to be seen today this could have been a life and death situation and I was told that I couldn't be seen this is not something that should have to happen to anybody who's seeking help from healthcare provider it's possible that other people have been turned away at the door needing medical attention it is not right and something needs to be done that requires them to fulfill their obligation to stay open to give people the care and attention that they need until the time that they're done maybe even overtime.

      Sea Mar has been receiving negative reviews for years and there hasnt been any change. Sea Mar provides medical and behavioral care for people who are low income/have state insurance. This is the only clinic that majorly accepts this insurance in my area. With that said, I have had to fire them due to feeling unsafe being treated by them. I have no other medical options now. The Drs push medications without treating the underlying issue, they are booked months out, the reception team make it impossible to reach the medical team or to be seen. My appointments have been not scheduled but told it was, scheduled incorrectly or Ive even had a Dr no show on me. The reception team also have a history of snapping at patients over the phone and even hanging up on me. I was even transferred while I was mid sentence today. I have spoken with numerous supervisors who end up arguing with me that their staff are fully trained. I waited months for a referral and told it was never in my chart but it was in fact there after all. I have been marked as a no show to appointments I arrived for but the dr no showed. After hours lines dont communicate with clinic. Dr tried changing psych meds Ive been on for years within the first 10 minutes of meeting her.

      I have had Depression/anxiety for about two years. I saw ***************************, a psychiatrist, Sept. 1st. I told her that I had drug-resistant depression for ********************************************** find other viable solutions. All she wanted was to prescribe MORE DRUGS to me. I wanted a letter of reference for ECT and for ketamine treatment. Both are statistically MORE EFFECTIVE than the usual pharmaceutical drugs that are offered. She totally refused to help me and even refused to write a letter of referral so that I would become a 'legitimate patient' in the eyes of these hospital and medical centers that offer these treatments. Her services were horrendous and she had no interest in helping me. I had an appointment with *************************, a psychiatric nurse, on November 30th. I tried to connect at 3 p.m. several times on Zoom, as I was supposed to. She disconnected me every time. Finally, I get a call from a secretary at 3:15 p.m. stating that she wants to do a screening before my session and then she proceeds to ask me my name, birth date, and the medicines that I am presently taking. I tell her this is all redundant information and *************** has all this information at her disposal and that we had gone over it during our last session. The secretary then says I will tell *************** that and implies that our session will begin shortly. *************** then never connects with me nor does she answer my questions when my appointment was canceled. Later, she says that I did NOT go through the screening interview and therefore, I was NOT eligible for our session. I retorted that this entire treatment was both unprofessional and unethical, and that her secretary should have told me explicitly that my session was being canceled. I contacted the director of the program in *********, and she just said, "What do you want me to do?" I said it was self-evident. I am seeking to contact the head of Sea Mar in *******.
