    Subject: Urgent Matter Regarding Discharge and Equipment for ************************* To Whom It May ********** 1/13/24, ************************* was discharged from Select Specialty Hospital following treatment for a leg infection. While I will refrain from detailing my concerns about his treatment during his stay, it is imperative to highlight areas for improvement.My primary concern pertains to the **************** particularly with regards to *************************. It was understood that upon discharge, an EZ stand and a lift chair would be in place to assist *******. Unfortunately, this has not been fulfilled. Despite his ability to pivot and transfer, additional assistance is required for him to stand. We were assured that **** would collaborate with insurance to facilitate the placement of these orders and have everything arranged before his discharge. Given *******'s extended stay at Select Specialty, it is perplexing that this has not been accomplished. I seek clarification on whether this lapse is attributable to communication challenges or operational inefficiencies. The burden of resolving this matter has now fallen upon his social worker and family, jeopardizing their home health support due to concerns about his safety at home.I urge your immediate attention to rectify this situation and ensure that the necessary measures are taken promptly. Your prompt and comprehensive resolution to this matter is greatly appreciated.Sincerely,*************************
