      I made an appointment for an annual eye exam and provided my vision insurance pre-appointment. They requested my medical insurance, which I declined to provide. I specifically advised that I only wanted services covered by my vision insurance. Upon arrival on the date of the appointment, this was reiterated. Throughout the appointment, several additional services were offered with explicitly advisement they were outside my insurance coverage; accordingly, I declined them all. At the end of the appointment, I was presented with an additional bill for $140 due to a test outside my vision insurance. This was done without my authorization and against my repeated, express instructions. I have advised the business of this numerous times yet they continue to pursue the charge.

      Complaint stems from additional medical testing that was performed to ensure medical diagnosis at the time of exam. Medical record documentation shows subject was told about diagnosis right away with verbal confirmation given from subject about understanding of diagnosis. Subject was told at time of check out that, this balance can go out to medical insurance due to confirmed medical diagnosis, there is a chance of a copay which we would bill at a later time. Medical insurance was not given at that time and patient left clinic. Multiple statements were sent to remind of balance on their account. There is no record of phone calls via phone number we have on file (or phone number used during recent conversation) calling the clinic to resolve balance, nor do we show any traffic on profile (all phone call/text messages sent to clinic are recorded and documented with our systems). Since balance was not collected, medical insurance not received, nor communication received about resolving balance on own terms we started balance collections process. Collection letters were sent to information that we have on file. The clinic was contacted on May12, 2025 regarding prescription, after verification process subject was told about collection balance needing to be collected. Receptionist who took the phone call was instructed that subject was never going to gave us medical insurance for balance, then hung up.
