    In April 2023 we received services at Healthy Smiles Dental. The people receiving services were me (Lynden) and my 4 children. My youngest 3 children have health/dental insurance. I paid for my oldest son and myself in full, and the co-pay for my youngest 3 children. The business took our payment and submitted the claim to insurance. After a little over a month the business started contacting me saying I owed the co-pay uncovered by insurance. I showed them that I had paid on credit card. They said OK. I thought it was concluded. A couple months later they continued calling and sending bills for the co-pay they said I owed but that I had paid. I told them again that I had paid and reminded them that I showed them the credit card statement that I had paid. They have continued to harass me including calling repeatedly through the week and sending bills in the mail. I most recently got a message that the debt was going to be submitted to collections. This feels like fraud of some sort.

    I have attached two word documents, one with the information we have for ****** ****** and her children, and one with our Guarantor notes regarding her account. I have also attached our Electronic Statement Submission History that shows all of the statements we have sent, and how they have all been opened by ****** and the links were clicked for her to see the amount she owes. We do not feel we have been committing anything fraudulent with her as our patient and we have made many attempts to reach her and collect payment on services that she had completed for her children in April of 2024.

    In 2023 my previous dentist stopped taking my insurance, so I began going to Healthy Smiles Dental because they did take my insurance. I was contacted by Healthy Smiles Dental that I was due for a cleaning, so I made an appointment and went in. A new dentist had purchased the practice and during my appointment I was told I needed a deep cleaning and a filing done. after the procedures were completed, I was instructed I owed $433.40 which I paid while in the office. about a month or so later I received an additional bill saying the new dentist isn't in my insurance network and that I owed approximately an additional $1700. I feel this is very misleading, I believe I should have been told at the time of them scheduling my new appointment that the new dentist isn't in my insurance network, I never would have scheduled an appointment with them had they divulged they were not in my insurance network, the only reason I started going to Healthy Smiles Dental was because they accepted my insurance in the first place. I honestly think it's unethical to perform services that are thousands of dollars of out-of-pocket expenses for someone without letting them know that their insurance is no longer in network before providing the services. I never would have used their services had this been mentioned. Anything you can do to help resolve this is much appreciated. Thank you
