      I received a billing notice from ******** for a doctor's visit from months prior. At the time of my appointment my health insurance showed that the doctor I saw was in-network. Somehow the facility itself was not considered in-network and so I was sent a bill. I called them asking about the bill and they informed me of the mismatch. They said that they would send it in for review and that I could disregard the amount due. Months later I got another notice saying that I still owed them money. I called them up again and explained the situation. They apologized, and again told me to disregard the letter and that they would take care of it. Today, I got a notice that the amount I owe will be sent to collections soon if I don't pay it. I tried calling again today stating how important it is that I get to the bottom of this and they again said they would look into this and get back to me. There seems to be no means of communicating with an authority there and I would just like for this issue to be resolved and not have anything sent to collections.

      I was billed for a procedure that was not done on me. I have reached out to the company to have the charge removed and all they did was ask me for my insurance information so they could pay for it. Again this procedure was not performed on me but they are billing me for it anyway. All I received was an x-ray. The dentist didnt even give me a proper exam. Today Im receiving the same bill from them again after I have told them that the procedure was never done.

      In January 2022 I went to the ************* at SunLife Family Health Center. I had a complete exam and was told I need three cavities filled and two Caps on my front teeth because the tooth was to weak to fill the cavity. I prepaid the total bill of ******* that day. with my Mother ************************ **** credit card. I went back in February 2022 and had two of the cavities filled 345 .was deducted from the **** I already paid. I had a credit for **** prepaid left on Dental. In February 27th 2022 I was admitted into ************************* for severe corrosive eshogitis and bone cancer. After being discharged from the hospital I personally went to Sunlife Family Health Center ************** to cancel getting my other cavity filled and Cancelled having Caps put on, due to the recent diagnosis of cancer. I asked for them to refund my Mothers **** Credit Card for the remaining ****. The dental office said yes they would call accounting and take care of the refund to the **** credit card. I have text messages and have called every month since March 2022 requesting for the refund of **** to be credited to **** CREDIT CARD. I have spoke to **** in finance in Oct 2022 who promised me that my mothers **** credit card would be credited within 3 to 5 days. NOTHING was credited. I tried 5 times to reach **** this month and was told she was in a meeting or out to lunch and that she didn't work their.

      In January 2022 I went to the dentist office for a cavity at Sunlife Family Health Center Casa Grande. They did an exam where they said in needed 2 cavities filled cleaning and 2 front Caps on my teeth. total cost would be ****. I prepaid for my dental work with my Mothers charge card. I had the two cavities filled for ****** in January I can't remember the exact date. I still needed to go back for dental build up for the caps and the caps to be put on. I never went back for the workup or caps to be put on. I went to the dental office in person telling them in February that I was diagnosed with Cancer and I would not be getting the rest of my dental work done so could they please refund my Mothers credit card. ******* has still not credited my mothers charge card. I have called and texted the dentist office every month for a refund and they say they will but don't. I spoke with **** in ************* in November where she told me that the medical part of Sunlife deducted ****** dollars for a past medical bill I didn't know about and that my refund to my Mothers credit card would be ******* and that it would be credited to her card within a few days. My mother has not been credited anything to her credit card as of December 16th 2022. I have been calling and **** and she does not return my calls and is out to lunch or in a meeting when I call. Can you please help get my Mothers money back for services never rendered but paid for. Thank You for your help Sincerely ***********************
