It started on July 18 when they gave me someone out of my network. Thinking that the person who they set was in my network. Not knowing because they told they was at first. I paid them $300 for the surgery then they turned around asked for $380 for the mistake they made. My old PCP and surgery group. Now I want my refund because they didn't do the surgery and I shouldn't have to pay for their mistakes when they send me to the wrong provider. I only paid for surgery nothing else but they holding my $300 dollars because they said they want me to pay for the $380 for the mistake. Im not paying that because it's their fault not mines. Now I want to *** because they didn't tell me they cancelled my surgery on 8/2. I went up there. Wasted money to get to the hospital. Now I want to *** for negligence, loss of wages, emotional distress, and pain and suffering because I am in pain. These people are rude and should be out of business for bad reputation now. Now I want $50,300 because they did be wrong plus I want to *** for $50,300 for negligence, loss of wages, emotional distress, and pain and suffering because I am in pain. We gone have to go to court ASAP if they do not pay $50,300. No surgery was done.

Mr. ***************** Superintendent Arizona ********** of ********* P.O. Box **** *******, Arizona ***** Dear **************: I have received correspondence from the *********************, dated August 22, 2023, stating that The settlement in the Sweet ****************** class action was between the **** ********** of ********* and borrowers who filed an application claiming a borrower defense to repayment. ********************* is not a party to the case. The settlement requires the ********** to take certain actions, including automatically discharging certain qualifying student loans and adjudicating other pending borrower defense claims. The settlement does not require any action from schools. Please direct any further inquiries on this matter to the **********.This letter is requesting an immediate discharge of my federal student loans associated with the ********************* to be deleted from my credit report tradeline associated with the discharged loan(s). I am a member of the class of federal student loan borrowers covered by the recent settlement of the Sweet ****************** ("Sweet") lawsuit. As of the above date my federal student loans are a total of $202,695.00. This student loan debt has put me at a huge disadvantage in purchasing a home because of my debt-to-income ratio for over the years. The discharged federal student loans need to be removed from the 3 major credit reporting agencies: Equifax, Experian, and TransUnion. I am looking for correspondence from you confirming the above request has been completed soon. ***************************** Register

P.O. Box ***** **********, Arizona ***** August 24, 2023 The Better Business Bureau **** ******************** *******, Arizona ***** To Whom It May ********* am very unhappy because I was never given the results about my diagnosis, treatment, and prognosis of my colonoscopy performed by ************************************** (Arizona Surgical Group, ***** **************************, #***, *******, Arizona *****) on January 25, 2023. I have never talked to the ****************** after my procedure about my biopsy polyps results or what the next plan of action for my medical care will be. I have contacted the doctors office on many occasions. I was never given the right to review my medical records with the doctor. Please note that I am at high risk, my Mother passed away from a colonoscopy performed in 2003.I was not treated fairly and respectfully by ****************** and her staff. The doctor has very poor bedside manners. She left the hospital after performing my procedure without discussing it with me. The office charged my credit card for another patients visit and gave me the patients medical records. The doctor and her staff would refer to me by my first name and not Mr. ********* All of the other patients were referred to by Mr. or Mrs. and their last name.I have the right to know what the doctor will charge for services and their payment methods. As of June 2023, I have been receiving a lot of hidden charges from the doctors staff ****************** for service of my January 2023 colonoscopy since I have been calling the office.I am very disappointed with the quality of care I received from ******************* I am requesting a full refund for all of my copays and my insurance company for the entire procedure. I have paid for a procedure and I didnt gain any results from it. I have requested, on many occasions, for a copy of my results from the office and I have not received anything. Im not confident the polyps were removed from the procedure.Cordially,******* Register, MSA, MS/CIS

My husband, ***********************, was seen at Surgical Group of ** on 3/3/23 by ***********************************. He found Surgical Group of ** on the Provider Directory of our insurance company. He was seen by ******************** for a damaged Achilles tendon. During the office visit, ******************** did a physical examination and recommended that he get an x-ray and MRI of his right ankle. He left her office and scheduled those appointments. He then received a bill from Surgical Group of AZ for $913.00. When I called their office to question the bill, I was told that insurance was in process and to disregard the bill until the claim had been finalized. When my husband returned to see ******************** for his follow up visit to get the results of the imaging on 6/1/23, he was taken into the billing office and told that if he did not pay the $913.00 at that time, that not only would he not be allowed to see ******************** to fix his Achilles, but that he'd be turned to collections as well. He said that he didn't understand what the bill was for because he paid his co-pay. He was told that the bill was for an ultrasound. He did not have an ultrasound. He was told that he had to pay the bill anyway. Being in pain and wanting to see the doctor to get help with his pain, he paid the bill. When he saw the doctor, he told her that he did not have an ultrasound. They could not find any images of the ultrasound, confirming that he did not have it. As of today, 7/26/23, we have still not received a refund for services that he was forced to pay for that he did not receive. Being forced and manipulated into paying for services not received is fraud and theft. The office says that they can't issue a refund because they're resubmitted the claim to insurance. But again, he did not have an ultrasound so we do not understand what they're resubmitting for. And when I called the insurance company, I was told that there are no pending claims. We need help in getting refunded since they will not issue the refund.
