      Centerlake imaging in *****, ****** and other medical centers are not able to fix the problem with their phones and mail boxes are filled. I need a mammogram very soon as I have a lump on my breast

      I have been trying to schedule an appointment with Centerlake and all the phone numbers go to a voicemail and nobody calls me back. I need a CT scan and the order has been over a month ago and it will expire soon if I dont apply. I am wanting this issue resolved with a phone call to schedule an appointment not a message machine. I find this poor medical attention.

      The business asked for my co-pay in the amount of $65.00 on services date 1/30/2024 for x-ray procedure of my knee at Centrelake Imaging ********************************************************. I contacted my insurance and the insurance had indicated that I have ********pay. ****************** also talked to Centrelake billing department that I have zero co-pay and /or any deductible and they should refund the co-pay collected.I have contact Centrelake billing department serval time and they do the go round sending my request to this and that departments. Until now I have not heard from them.

      I went to Centrelake Imaging in ******, ** at ******************************************************************* on October 14, 2023 for a *** of the right shoulder. They charged me $100 before the *** which I paid. I received the attached letter from my insurance company saying that I should NOT have been charged the $100. I mailed a written request to Centerlake Imaging at ************************************************************* on December 12, 2023 with a copy of the letter from Anthem and asked Centrelake for a refund of my $100. I have not received a reply from Centrelake. I am asking for an immediate refund of the $100.

      I went to Centrelake's ****** location (*************************************************************************), phone number ************** and received imaging services on 7/13/2023. I received a collections notification in my mail from ************ on 12/4/2023. During the period between the date of the imaging service and the receipt of the collections mail, I did not receive a single notification from Centrelake ****** that I had an outstanding balance for services rendered. Upon contacting Centrelake ******, the billing department staff stated that they attempted to mail the bill to my address 3 times (August, September, October). All 3 times, I did not receive the bill. I requested that they email me all 3 billing attempts which they stated they would. However, I had to follow up a 2nd time and remain on the line with the billing department staff until I finally received an email. The billing department staff also confirmed that the billing address they had on file matched with my home address. They also stated that I did not have a phone number on file which is false. I provided a phone number during registration for my procedure. The billing department then claimed that the bills were most likely lost on ***** end. I believe that the 3 bills they provided me could easily have been backdated. I find it difficult to believe that with the correct mailing addressing, **** could lose my bill 3 times in a row. I would like for Centrelake to reach out to the collection agency, ************, to correct this issue. It is completely unacceptable that my credit be negatively affected by a situation that I did not contribute to. Had I actually received the bill that they stated was sent 3 times, I would have paid the balance.

      Hello, January 2022 I got a service done by Centrelake imaging after I was told by The Upland womens health clinic that I needed further testing. The womens clinic that sent me to get imaging done mistakenly took my insurance not knowing I needed a referral. They found this out after my service from Centrelake imaging was already completed. The ******************** apologized and they waved my service. All throughout 2022, I was in constant communication with Centrelake imaging to see if they can do the same and wave my service or at least work with me on a payment option. I called about 15 times all throughout 2022, each time I was told they were still working on my case. On March 8, 2023 I was verbally told that my service was going to be waved. I never received a bill in the mail after that or any indication that I still owed. All the sudden, on September 27, ********************************** the mail. I did not receive a final notice, or any sort of verbal notice from them. I have called Centrelake 25 times since then. I have documented everything. Each time I call, they refer me to their billing department. I either speak to ********************, or *** who say that this can only be resolved by a manager. The issue is that they cant connect me to a manager. Each time I call, the manager is unavailable. They cant provide me with any time frame I should call or a managers phone number, and they themselves say they cant even reach the manager. I have taken multiple hours out of my personal work day to get this resolved but they are being very unethical. I believe I am being tested very unfairly, and this is not a ethical business.
