    On 5/2/23, I went into Adventhealth Imaging Center of Altamonte Springs for a for a mammogram and ultrasound. My health insurance was with Cigna. Before I made the appointment, I went on to Cignas website and chose a facility that was in network. On the day of, I paid my copayment and was not made aware I would owe anything else. Several weeks after the appt, I received a bill from adventhealth for $555.47 and $255.00. When I called and inquired, my insurance company refused to pay the in network amount and I was charged as out of network. According to *****, Adventhealth did not submit the claim as being from the center, but as being from a specific doctor that is considered out of network for me. I went back and forth for the past year calling advent and ***** to resolve this. I just spoke with advent again and they will not change the coding to the center instead of the specific doctor name. I am frustrated because I believe I did everything right and should not be paying out of network costs for those imaging appointments. ***** has told me there is nothing they can do until advent shows I went to that center and not that specific doctor.

    On Jul 27, I went to Advent Imaging Altamonte Springs to have a CT scan for upper epigastric pain. I hand carried the referral that said family history of pancreatic cancer, elevated amylase and lipase, and epigastric pain. On my records, Advent Imaging typed family history of Human immunodeficiency virus(HIV), abdominal pain, pit of stomach etc. This is clearly not my visit summary. I notified them immediately and they added a line more to the wrong information but didn’t put my information in. I am concerned that this is on my records and could effect my treatment. According to Advent Health this violates my patient rights on their own forms #30 A patient has the right to confidentiality of his or her clinical records, and to access, and request amendment to, and obtain information etc.as permitted by law as quickly as the record keeping system permits. The Manager Christena R******** has never contacted me even though Advent Health told me several times she would. On August 10, I asked if there was someone else who could help but the Advent Health advocate Jennifer keeps telling me that they are working on it or they have fixed it.
    I have emailed, called, and filled out formal amendment paper work. called , and emailed was Denise S*****, Advent Health CF Data Integrity Specialist HIM operations.
    The Clinical Manager Christena R******** has never contacted me even though Advent Health told me several times she would. On August 10, I asked if there was someone else who could help but the Advent Health advocate Jennifer keeps telling me that they are working on it or they have fixed it. I checked my file a few minutes ago Friday October 20 and it is still not fixed. I have over 30 texts I can show someone but it has not been fixed.

    After filling this out and months of asking for my records to be fixed, they finally corrected it. That is all I wanted!
