    I dropped off a medical form for my daughter, Heaven Washington, to be filled out Feb 29, 2024. They told me that day I dropped it off I did not need an appointment as her last medical review was done 3/16/2023 and they are good for 1 year. The office stated it will take about 3 business days to get the form completed. I went to the office on 3/5/2024 to pick up the form the lady at the desk gave me the form back not filled out. She didnt mention it was unable to be completed until I looked at it and stated to her it wasnt filled out. She then looks at a sticky note attached to the form stating an appointment would need to be had because the provider that was going to file out the form hasnt seen my daughter (note she is new to that office). Today, 3/6/2024, I went for the appointment with my daughter and tried to ask the provider who stated I need to have an appointment why and once again I was told because SHE never saw my daughter. While there are 2 other providers in that office that has including ***************** who initially diagnosed my daughter with the **** and ODD. I was forced to pay $106 to go back there with the provider who then filled the form out from the information in the computer all while not even looking or speaking to my daughter. I would like a refund due to the fact there was a provider there to fill out the disability form (all about past history), all the information was in the computer and the fact that she didnt even check nor speak to my daughter about anything.
