    I was forced to pay $106 sick visit to have a medical form filled out about my daughters diagnosis date of ADHD and ODD that the office in ********* did in 2020. Her last well check visit was done 3/16/2023, they are good for 1 year (until 3/16/2024) Dropped off the form 2/29/2024 which the lady at the desk looked up my daughters information and stated she DID NOT need an appointment since she was up to date. Fast forward to 3/5/24 when I went to go pick up the form and was told that the provider to was going to fill out the form wanted to have an appointment to see her since SHE hasnt seen my daughter before all while there are 2 providers in that office who has. Paid the $106 under duress because I needed the form for my insurance went to the back with the provider who never checked or spoke to my daughter and filled out the information FROM INFORMATION FOUND IN THE COMPUTER.
