Thats over 2 months ago and the dental office submitted the information to Delta Dental close to that date and I also submitted the requested information after that to Delta Dental. I think over two months to process this claim is excessive. And now theyre asking for 7 to 10 more days.

I would like to file an complaint for denial of service because my doctor got an explanation of benefits for my daughter ******* ***** prior to our procedure visit on November 21, 2025 that showed the services were covered and insurance would paid a minimum of $810, but Delta Dental denied to cover the claim afterward and paid absolutely nothing. Here is a response from the doctor, Dr. ******** ****, expressing surprise that the claim was denied. "To the best of our knowledge, Delta Dental denied coverage because the procedure is not covered under your plan. This was also a surprise to us, as it contradicts the information listed on your Member Benefits page. As your Member Benefits, which indicates ************ services are covered at 50%."I have attached several documents that show the contract benefit for the listed services of 50% or 100% for the listed services, but Delta paid nothing. This contradicts the prior authorization and the claim paperwork itself. The Delta patient portal also shows an out of packet costs of only $335.50.Business Response

The cost estimator shows dental procedure codes d7140 as covered services, the dependant did not receive this specific extraction. The submitted codes not covered were d7240. It is always best to call in and check information or have any procedure pre authorized to be sure services would be covered.

I am unsure where you saw 100% of contract benefit level. The attachments you sent do not show this. It shows a different procedure code than what was serviced for your dependent. In the future we highly suggest a pre-estimate for a major surgery unless your provider gives you the procedure codes themselves, looking online and assuming all oral surgery is the same is not recommended.

Delta Dental is trying to charge me extra payment ($42.25) to continue receiving my dental benefits. I pay $42.25 each month for a DPO plan for the state of Texas including adult orthodontic treatment. I have received several invoices starting in November 2024 that my DPO Plan of the State of Texas is cancelled as of December 2024. Dental ProviThe Dental Provider's list includes ********************** The dental clinic assured us they were contracted with Delta Insurance and that the procedure would be covered. However, after my daughter had a tooth extraction on August 11, 2022, Upon checking online we were shocked we have a bill for $1,496. When my daughter called Delta on January 3, 2025, she spoke with *******, who advised filing a grievance and mentioned that the dentist was not contracted at the time of the service. Following the insurance company's advice, my daughter filed the grievance, yet now Delta Insurance claims the 180-day filing period has passed. I find this situation unfair. The purpose of enrolling in Dental Insurance is to reduce costs, not to face a financial burden. Delta Insurance is only willing to cover $369.60, which is not a fair contribution at all..I want Delta Dental to cover the Insurance bill. ***** ***** Id number: ************* Claim number: ************** I went in for a crown on Aug 16th 2023, however, the doctor wasn't able to put in the crown due to a bite issue and referred me for crown lengthening. During this time, I found out I was pregnant and didn't want to get any invasive dental procedures done due to my high-risk pregnancy. I unfortunately lost the babies due to the high-risk pregnancy. Due to my health condition this is why the claim is over 180 days old. After the grieving period, I went back to them for crown lengthening consultation as I was advised by Dr. ******* the doctor there told me it would be a 50/50 chance of working so I decided to have tooth no. 18 pulled out altogether. However, doctor (******) still charged me $533 for a crown I did not receive which is to me is considered fraud. If I did not receive a product, how could you charge me and my insurance. I chose this dentist because of the expertise of the dentist and the difficulty of the crown, knowing that I would only receive a portion of my cost from Delta Dental. However, as of this date and after numerous phone calls to Delta Dental and my provider, I have still received no portion of the payment I made to ***************************** for the crown.From what I have been told (and I have received different explanations from Delta Dental every time I have talked to them) the problem is that ***************************** did not complete the paperwork properly. My daughter had her routine two teeth cleanings and one initial eval by an orthodontist this year. Delta dental is claiming we went over out D0120 and D0140 uses. My other daughter had the exact same evals done inn 2018 and everything was covered. I have had numerous **** with delta tell me that this should have been covered. It has been resubmitted numerous times on my behalf only to be denied. I then filed a complaint against delta dental and did not get a response. I finally emailed them again today and was told that its not covered. I have proof as to my oldest daughters being covered for the EXACT same things. NO policy changes were made. We have the exact same insurance. Why is my youngest not covered? Delta dental is wrong and needs to refund my $49 they owe me! There is no way that someone can't not get their two cleanings yearly and go to an orthodontist as well and go over the allotted visits. This did not happen to my other child yet its happening to this one. Delta dental is in the wrong. This has been an ongoing issue since July and there are plenty of notes that can be gone over. Your **** need proper training if they were in the wrong by telling me what thy did, numerous times. You also need to revert back to my other daughters claims to see they were all paid in full yet this same exact occurrence is not.Business Response

After further review the claim for 7/25/24 for ******* ********* is processed correctly. Your plan allows to exams per year, there was an exam done 1/16/24 as well as 6/6/24. The claim for July would be considered a third exam and therefore would not be covered. As far as your other daughter there is no record of her having three exams in 2018, she had two regular visits that were covered.
