I am a retiree. My last employer provided retiree dental insurance thru Delta Dental (DD). For ************************************ the fall of 2023 I called Delta Dental to sign up directly. I was told by the Sales rep that my plan and coverage would be THE SAME as the benefits I had received from Delta Dental thru my previous employer though my premium would be much higher. I was told that our 'preventative services' - check up, cleaning and bitewing xrays -- would continue to be covered 'close to 100%' as they always had been , though she said closer to 90% because we go to an out of network dentist (and always have while w/ DDl) In the past, with DD, the most we've ever had to pay out of pocket with DD for preventative services was around $20-$30 (which indeed calculated out to about 90% covered). So, we get our first bill for 2024 for both of our check-*** and DD only covered 42% of the charge (paid $133 of the $316 bill for each of us). I have received a total bait and switch offer from the sales rep. I was very clear on our circumstance asking if it was the same plan with just a higher premium, and I was clear we always use an OON dentist. When I called DD they just said, sorry costs just go up every year. There is NO WAY they have had a 500% cost increase and there is NO WAY this is the same plan we had before as I was told. via the sales ********** we will have 4 preventative visits (2 each this year), our 'benefit' from DD will total $532. If what the rep had told me was correct and 'around' 90% would be covered, they should be covering approx. $284 for each visit (not $133). I am seeking restitution of $151 x 2 ($284-$133) for the two prev. appts thus far, as well as the same for appts we will have in the fall. (OR, they can drop us from policy and reimburse me for the premium we paid for the yr ($997.84 - $266 pd = $731..84).Business Response

After I filed this complaint, I was able to contact Delta Dental (very difficult as no customer service phone # is to be found anywhere), and was told I could cancel the remainder of my policy. Delta Dental has agreed to do so and refund me for the balance of the year (as I paid our entire premium in full in late 2023 for 2024 coverage). I was told a check will be sent to me for $621.38 with the cancellation effective as of today. So, I am expecting to receive that check in the next 7-10 days.
I would like to revise my request therefore to properly cover 80-90% of the preventative services we received thus far given I was told by the ** salesperson to expect approximately 90% coverage for out of network preventative service. ** has only covered 42% of each claim. Each claim was for $316 for exam, cleaning and bitewing xrays. 90% of $316 = $284.40 vs. the $133 which was applied to each bill. Therefore, we are seeking $154.40 for each claim ($284.40-$133), for a total of $308.80.

I have reviewed the response made by the business in reference to complaint ID ********, and find that this resolution is satisfactory to me.I appreciate BBB's very rapid response as well as initial follow-up from Delta Dental. I am still very unhappy with not receiving the coverage I was told we would get at the time of purchasing the policy. However, because I was able to cancel the policy and get a refund (and subsequently only be out a little over $100 vs. $400+), I am dropping my complaint as I don't have the time or energy to pursue further.
The cancellation of our policy was pursued directly with Delta Dental. If I do not receive my refund in a timely manner as promised, I will be filing a new complaint. However, hopefully this promise from Delta Dental will indeed by met.

I needed to have different filling to be done by my dentist. Knowing this, I specifically increase my dental plan to the maximum possible where filling are covered 100%. My dentist did an insurance pre-estimate. Upon receiving the pre-estimate, I was shocked by the out-of-pocket amount I still needed to pay for the filling (roughly 50% of the cost).I contacted 3 times Delta Dental in 2 months to have more clear information. I was told an error was made by their system on estimating the cost of the pre-estimate as well as the subsequent dentist claim, and that the out-of-pocket amount I would pay will be lower, and that they will correct the claim and resend it. However, after another call, and speaking to a manager, I was told that the amount on the claim was correct. That because I chose white filling, and not the regular silver, and that I live in MN, dentists were entitled to ask me more money. However, my dentist says he never heard of that and that it was not the first time he heard some conflicting messages dental insurances send to either the patient or the dentist. I filled a complain to Delta dental explaining them that I was repeatedly told by their people that there was an error. The insurance answered the complain saying that I should check myself on the coverages I have and that was I am told verbally on the phone by their people is not binding, and therefore they are not at fault.I am disappointed by the lack of ownership from the insurance. I told them if the amount I needed to pay would have been confirmed from the beginning, I would have been ok, and financially prepared (my choice to continue or not with white filling), but because of their repeated guaranty on the phone that an error occured on the estimate and claim documents, I was not financially prepared for the $900 I need to pay while covered at 100%. I am rejecting this response because:The pre-estimate was prepared for a series of filling to be done, with a total of around $900.
I did the job in 2 times, resulting in 2 different claims: one of ~$400 and the other one of ~$500.
But the problem I faced is the same for all these filling where I got white composite and not silver. Your colleagues on the phone assured me several times that an error was on the pre-estimate (and therefore, would appear on the subsequent related claims), and that I would not have to pay that amount. You can hear that on the recordings.
Insurance information, benefits, what is covered or not are always very obscure to understand (on purpose or not...). You have a phone number available on pre-estimate and claims documents that we can call to ask for precisions and help to understand these documents. We (patients) should be able to rely on what the people on your Hotline explain to us.
I am very disappointed by the letter I received from Delta Dental saying that you would not revise the amount I have to pay because I should not rely solely on what your colleagues on the phone said, and should understand my policy. That is a very unprofessional response.
Furthermore, in your letter, you are mentioning not choosing what is taken care or not in the insurance. My dentist explained to me that the important majority of people do not make anymore silver filling on their back teeth nowadays and they do not understand how an insurance is not covering what is mainly done by patient. Patients like me are paying an increased premium plan (and not the basic one) to have a 100% coverage specifically for this job to be covered, and end up paying close to 50% of it instead.

Delta Dental (through the *** marketplace) somehow sold me a "kids" dental plan which was not clearly or obviously marked as such (or I would have noticed). When I got a crown (at Dental Depot in ****************, **), they declined coverage because "exceeded the age limit". My dentist (Dental Depot) said they had never seen that reason before for a crown. My dentist office called Delta Dental and my dentist office said that the Delta Dental representative said I had a kids or pediatric plan. This resulting in a surprise dental bill of over $400.I have contacted Delta Dental (on April 16, 2024) and they have not (as of yet) been able or willing to fix this issue by changing my dental plan (retroactively to March 1) to one that is appropriate for an adult individual.Customer Answer

Being covered by Delta Dental, I went to a local dentist to get an exam and a cleaning. one. That dental clinic, however, refused to do a proper exam (the dentist looked at my teeth for less than 2 minutes) and also refused to do a standard cleaning--instead trying to get me to sign up for unneeded services with an out-of-pocket pay of $1,000. Among other items, the clinic insisted that I get a deep cleaning for inflamed gums but refused to bill my insurance for the one that Delta Dental states that it covers. Delta Dental lists this office as a covered provider even though the reviews of this place all express clearly that the dentists here run a predatory business model, refusing to bill patients' plans and aiming to sell unneeded procedures with high out-of-pocket pays instead.I then had to go to a second, much better, dentist, where I received a proper exam, cleaning, and the care I needed. I also received a referral to see a periodontist. The issue, however, now is that, according to Delta Dental, my consultation with the periodontist (who is covered by Delta Dental) will be treated as an exam--and that I have already maxed out the two exams that I am eligible for. I contacted Delta Dental's customer service several times but was told that there is nothing they could do about the dental exam that was charged to my insurance account--but that clearly did not happen. I was told that, as soon as a dentist looks into my mouth, even if only for a second, that counts as an exam. Which to me is unacceptable.Business Response

Delta Dental hasn't paid out the full amount to my dentist in regard to a root canal and crown I had performed at the end of 2023, Advanced Dental Care of ***********. I am receiving bills from my dentist due the fact Delta Dental hasn't paid out the full amount expected. I have called Delta Dental more than 5x about this, Delta Dental says I am only responsible for my copays, which is what I have already paid my dentist and nothing more. My dentist is saying I owe more Than the copays because Delta dental didn't pay the full expected amount to them. I have even had the dental office on the phone with delta dental at the same time and nothing is being done on either end. Delta dental won't pay the dental office anymore money. Delta Dental will NOT pay on any of my claims since September 2021, and is using bad faith tactics to accept premiums but not pay claims. A grievance was filed for all claims beginning in Sept. of 2021 when we married. Today I received a stack of claim rejections from Delta Dental NEW reasons: a policy stating its been 180 past the claims. They intentionally delayed applying info to claims to avoid payment. The orthodontic office also wrote to them with my familial relationships and have MANY Delta Dental complaints for the same tactics. Claims were filed with Delta Dental at the time of service. They had my dependents ID documentation. My employer was required to send to them when we got married. Which they did or they wouldnt be on my policy. We sent required certified custody papers, birth certs, marriage & divorce records, copy of Id and social security card with married name, and they claimed no receipt. We mailed a SECOND copy AND emailed a copy to an agents personal email who said she would apply it. We mailed EOBs to the address given by agent, to be told they never got them. We called over 12 times and asked for explanations. They said we have everything we need refusing ALL multiple requests to speak to a supervisor. They gave wrong fax numbers, so we called again and got ANOTHER wrong fax number. We have the last fax rejection. We asked again for an address to send ALL EOBs AGAIN as documented. Our oldest kids (active duty army dependents who are age-ineligible for military dental) gave permission to Delta to discuss their claims. Now, Delta says they never received permission, but they discussed the claims with us until we filed a grievance. Documented. We consider Delta Dental to be Bad. Faith. Insurance. due to the tactics used to avoid paying on $22k in claims. We want all claims paid with interest, and without ANY further delay attempts and will escalate it to any degree so less able people and other military children arent defrauded.Business Response

Since September 2021, Delta Dental has not paid out on a single claim for insurance that my husband and I pay for. We have four orthodontic claims for myself and three of our dependents and even more dentist claims that they claimed they needed more information on about four of my six children. I have gone to the courthouse and had certified copies of my marriage certificate. My custody papers, my ID with my new name, my youngest childs birth certificate and physically mailed certified copies of mine and my childrens personal information, and none of these documents have been returned to me, and they didnt apply this information to more than one childs account. THEN they claimed they needed proof of what our second dental insurance paid out, because they didnt know who was primary. I explained that THEY are primary, because my husband has held his policy longer than I have, and then one phone agent said No, we are not primary, your other insurance is primary which is wrong, but if they were denying the claim based on not knowing who the primary insurance was, then why did the agent tell me they are not primary. If thats the case, they would have paid as the secondary insurance provider. One claim alone for myself is in excess of ******. I have called over a dozen times, have been hung up on REPEATEDLY with no call back, have been told that speaking to a supervisor is not an option they provide, have been told a supervisor would call me back. And they have never called. And they have claimed the only thing they can do is just continue to reprocess the claims for over 2 1/2 years. I have tried to be patient with this process , but we are owed thousands for these dental bills that we pay Delta Dental to insure, and we are to the post of escalating this.Business Response

My daughter had her wisdom teeth removed on 11-11-21, at the age of 17. The claim was filed with Delta Dental of Georgia, but they claim it was denied because the *** needed to be updated; I did not hear from anyone about this...ever. In Jan 2023, I recieved a bill from the oral surgeons saying I owed this money; I reached out to Delta who said it had gotten "stuck in claims." They didn't mention the *** at all. A couple months later, I got another bill, so I called Delta again and filed a grievance; no one ever got back to me. A couple months later (May 2023), I called again, THIS time, they told me about the ***, and I sent everything they needed (divorce decree). In the meantime, the oral **************** was in contact with them for over a year, sending requested info 7 different ************ weren't asked for a divorce decree until May 2023 (same as me). When I called again in fall of 2023, it had still not been filed again and I spoke with a supervisor who said they'd take care of it. When I called again this month (03-2023), it had still not been filed again and it was escalated to management (3-13-23) When I called yesterday (3-23-23), I had to file with management of grievances.
