  They said DOS 12/06/2023. They used HCPCS code A4316 which Is an indwelling urinary catheter 3-way port for irrigation. They charged provider almost $2000. I never had an indwelling catheterever! Clearly fraudulent.

  6/21/24 Fraudulent scam against ******** and using my personal info, ******** account, to bill and obtain money from ******** and me. Charges for urinary supplies - not needed, never received. Got $1400. Out of ******** twice. All fake, scam charges. And the Dr listed is in ******* not in *******. Pull up the *** on internet and find him. His name is ***** something. Reported to ********, and ****** of ********* General.

  This company placed a fraudulent claim with ******** and my secondary health insurance company for $4,000 on two different dates. ******** was alerted as well as social security. This company is fraudulently billing ********

  I just received an Explanation of benefits from my supplemental insurance and discovered 10 charges to my account during the year 2023 in the amount of $1960.00 for each date of service. I was billed for unrinary catheter supplies that I have never used nor did I receive the items that were billed to my insurance. This is obviously a fraud case and Main Street DME should be held accountable and should be sent to jail. I am hoping that charges have already been filed against this company. Luckily, my supplemental insurance paid $0 but I will be checking my ******** to see if they were billed and if they paid anything. ****************

  We were charged twice for urinary catheters we didnt receive or need. I called my insurance and they said catheters are often sent through on fraud cases.

  This company charged my ******** $1960. I don't use their products and never contacted them to buy anything. The first time, ******** didn't pay the claim. The second time they paid the $1960. This is fraud and I don't know how they found my ******** number. They are scammers and thieves and should be shut down and sent to jail. I called ******** and my supplemental insurance and they opened an investigation. I would rate them as negative 1 star and give them an F.

  Why is this still going on? Its happened to my brother in law, a good friend and myself in the last year. These are just a few people i know it happened to. Weve all reported it and it obviously keeps happening. *** looked at the rejections by ******** for several months, then suddenly there is an approval and then another. Its a known fact that insurance fraud drives up premiums. Mine surely has. Without naming names and for as long as this continues to happen, I am beginning to wonder if there are more flies in the ointment. We might be surprised about what entities are behind this garbage. It sure reeks of a major, major ploy by some really slimy players. I say lets work together to put a stop to this blatant fraud.

  These scammers billed ******** twice for $1,960 for urinary catheters that my wife doesn't need and didn't receive. Supposedly ordered by a doc who practices sports medicine in ****, **, while we live in *******. In short, the same story that everyone else here is reporting.Why can't the feds shut these scumbags down and put them in jail where they belong?

  This company has obviously been practising ******** fraud. I got two letters from my secondary insurance regarding claims from december 2023 and January 2023. These claims for urinary catheters had been processed by ******** and paid (over $1400 each) and forwarded by ******** to my secondary who wanted more information from me before paying the rest. Since I had never heard of the company and had no catheter history, I have instructed my insurance company to treat the claims as fraudulent. I have also requested a ******** fraud investigation of the company to be pursued.

  This company has been billing my insurance $1960.00 per month since March, 2023 for urinary catheters and including Jan 2024. I don't use urinary catheters, have never ordered any, and have never received any from this company nor from any other company. Check your insurance statements carefully! Unfortunately ******** has paid them twice so far. They need to be prosecuted to the fullest extent of the law.
