      I visited the Endeavor Health Immediate Care in **************, ** on 2/26, 2/28, and 3/11/2025 for urgent care services. These visits were for a hand injury that required stitches and two follow-ups. My insurance with Ambetter of IL includes a $10 copay for urgent care. However, I was incorrectly billed copays and coinsurance on multiple claims due to inaccurate billing codes submitted by ****Each visit resulted in two claims: one professional (physician) and one facility - a total of 6 claims. Only 2 were billed correctly (the facility claims for 2/26 and 2/28) using Rev **** 0516 ************ which Ambetter processed properly with the $10 copay.The remaining 4 claims were billed incorrectly:All three professional claims (2/26, 2/28, 3/11) used Place of Service (POS) 19 Off-campus outpatient hospital instead of POS 20 *********** Facility.The March 11 facility claim used Rev **** 0761 Treatment Room instead of 0516, which caused Ambetter to treat it as a general outpatient visit, applying coinsurance instead of the correct copay.Ambetter confirmed to me that:Claims submitted with POS 20 and Rev **** 0516 process correctly as urgent care with a $10 ************ submitted with POS 19 or 22, or Rev **** 0761, are processed as outpatient hospital services and trigger coinsurance or incorrect copays.Despite providing the UB-04 and CMS-1500 forms proving the incorrect codes were used, **** billing department repeatedly insists that "coding was reviewed and is correct" but refuses to explain why urgent care visits were submitted with outpatient hospital ******** one at *** has offered a valid explanation or attempted to fix this. Ive made multiple calls and messages, and this process has caused significant stress and anxiety. Im filing this BBB complaint as a last resort.I am requesting that *** correct and resubmit incorrect claims using POS 20 and Rev **** 0516, and ensure Ambetter reprocesses them with the proper $10 urgent care copay.

      Endeavor Health (Evanston and Skokie locations) is in absolute crisis. Their doctors (of all specialties) are resigning and leaving the system left and right. As a long-term patient, I cannot get in to see any of my doctors because the waiting lists are too long, my doctors are all resigning too quickly, and then my new doctors are resigning and leaving before I can even get in to see and meet them. This leaves me (the patient) "stuck" between non-existing doctors (both past and future). This ongoing situation prevents me from receiving necessary and routine medical care, ongoing treatments and regular check-ins, and any preventative medicine I might require. Furthermore, this ongoing situation at Endeavor Health counteracts a healthy lifestyle for myself, it undermines my overall health (physical, mental, emotional, spiritual), causes me an extreme amount of severe undue stress (body, mind, spirit), prevents me from receiving any "continuing care" (as they love to advertise on their central call system waiting line), and it doesn't help to support a healthy lifestyle. My overall health is being undermined by being a patient at Endeavor Health on the **********************. I am being negatively impacted by my own health care system! Shame on them! How is this legal? How is this medical care? In the past one calendar year, I have lost 5-6 doctors total (both former and new). I currently am "in between" a PCP doctor, an Endocrinologist, and a Dermatologist (on my 4th one now). Endeavor Health's Board of Directors MUST do something NOW! "Profit over people" is NOT the answer! This ongoing nightmare of a situation has nothing to do with the amazing and talented doctors, nurses, office staff, etc. It is purely a "top-down" executive leadership and financial decision making problem. The overall health of the general public on the North Shore of ******* has been put at severe risk by this ruthless, unethical, and neglectful Board of Directors. Change is needed NOW!

      The service committed a coding error and refuses to acknowledge or correct it. My insurance carrier is working hard to resolve the issue, but four months later, it has yet to be resolved. I was charged $238 when the charge should have been $0.00.
