    I had an implant done with *************** in January 2021. It failed to take hold in June of 2022 and I had to then have the crown that I had paid for and placed on the implant removed due to the failed implant This was all done regarding tooth #3. In the meantime, I required the same work to be done on tooth #**. At that time, *************** offered to repair the work on tooth #3 and issued an estimate for the work on Tooth #3 at no charge if I paid for the work on Tooth #**. Estimate is uploaded. His office contacted my insurance at that time and determined the none of the work would be covered by my insurance. In January 2023 I agreed to go forward with the work and paid $5800 per the estimate. I still needed to pay my dentist for 2 new crowns. (1 which was previously paid for before the failed implant on #3. In order to get the insurance to pay towards the crowns; I needed NOS to submit to insurance showing the implants had been done. They billed my insurance $**,300 even though the estimate was for $5578, which I already paid at the time of service. Well, my insurance did pay them $2662.80 which I needed to pay my dentist for the crowns. This $2662.80 went towards my $3000 allowed for the year. I am requesting they forward me the money paid by my insurance as they should not have been paid since I paid 100% of the estimate at the time of service. This money should have been used towards my crowns done by my dentist (*******************) which was $5800. I feel this is fair and just as I now have purchased 2 crowns for tooth #3 which was due to the failed implant in the first place and paid the estimated charges up front on both tooth #** & tooth #3.

    The provider will not release my medical records without payment of a past due balance. I've sumbitted complaints that this violates HIPAA policy, but this is also impacting my ability to have dental work done in the timeframe needed to avoid additional costs.
