    A family member was a patient at a rehab facility undergoing physical rehab prior to returning to a private home where he lived by himself. During the rehab stay, he passed away unexpectedly. There were visual signs of a need for and a lack of urgent medical care being provided by the rehab facility. Forensic Mortuary Services was paid $3500 to perform a full investigation. The investigation was to include an autopsy with a preliminary report within a few days of the death to complete the death certificate, and then a final report that would take 1-2 months to complete and would include a review of all inconsistencies and abnormalities between the medical records provided by the rehab facility and the findings from the autopsy. Despite numerous phone calls and emails, Forensic Mortuary Services never performed or completed the full investigation, and subsequently stopped responding to any communication.A "one-star" rating was selected because there does not appear to be an option for "zero" stars.
