      My husband was charge for ***** test on his ******** on 4/21/23. Interesting as he passed on 8/2021 from Covid. They might need to get their information updated as we did not receive any kits as they claim they may have sent.

      Date of Service: April 17, 2023 Attempted to **** ******** $120 for a covid home test kit for a deceased ******** recipient. This information came to light when we received a ******** EOB for my mother-in-law. My mother-in-law passed away in January, 2023. So obviously we did NOT request a kit for her. We are not aware of a kit being received at any of her former addresses.This is an obvious case of fraud on the part of Cipher Global LLC, ***********************************************************************, 312.771.3491.

They are listed under various address my ******** summary notice shows above city and state. Address ********************************** dr **** ******* should be shut down for ripping off ******** !!

      I have never signed up for COVID tests and every month, I receive 4 tests, and Cipher labs bills ******** and ******** pays anywhere from $85 to $100. I have received so many that I have had to throw them out due to expiration. I have contacted ******** and they filed a complaint , but said they couldn't do anything else. I have tried to ***********,but have been unable to reach anyone to file a complaint or make this stop. This is fraud , pure and simple and I wanted to knsow if you could help stop this.

      My mother was billed $120 for a covid test on April 3, 2023 that we never ordered. She has not been ill. I am a physician and have tests available. This company is supposedly in ******** and we are in ********. Basically they committed ******** Fraud and should be closed down.

      Cipher Global keeps billing ******** plan of deceased person for providing an unrequested COVID test. Clearly a scam.

      I recieved a ******** summary notice for my father who passed away in 2021. ******** of course denied the claim but Cipher Global LLC, billed my deceased ******** ******** $120 for covid test. The claim was dated between April-June 2023, more than a year and a half since my father passed. I called the number on the statement. ************ for this company. It rings and the line is answered but stays silent for a long time, so I just hang up. Tried 3 times. I believe this to be an unlawful act of filing false claims.

      March 29,2023 they filed a claim with ******** and were paid for Covid tests. I did not order them, the local health **** has them for free. I remember getting some tests in the mail but since I did not order them, I did not trust them and threw them away. Do not know how these crooks got my ******** number but I have filed a fraud alert with ********. I get frequent robo calls about free tests but always hang up on the crooks. The phone number listed is not answered.

      Cipher Global charged ******** $120.00 on 4/13/23 on behald of my mother, *************************, who passed away and was creamted in February 2022. This company has committed fraud and needs to be held accountable.
