Company Cipher Labs Filed a Fake Claim with ******** on 4-20-23 Claim date 4-06-2023 charging ******** for a Covid-19 Test that I did NOT order and did NOT receive any test. I DO NOT WANT ANY COVID -19 TEST. ******** was charged $120 and ******** paid $96. ******** was the a victim. I am a victim also because they used my ******** Card and Social Security number. On 5/02//2023 another Company (***********************) filed a fraud Claim on 5/2/2023 for $96.00 and was paid $96 by ******** for a Covid Test-19 using my ******** number. This company charged me for two Covid 19 testing kits that were never ordered nor received. I contacted the company on March 7, 2023 and was told that I would get a call back that same day after they researched the issue. In the last month, we have received two separate packages with Covid test kits. We did not order these and do not want them. Cipher Global LLC sent me two home Covid test kits that I did not order/request. Cipher Global LLC also billed ******** under my name without my authorization. This is ******** fraud! I do not want these tests. These tests expire on 25 APR 2023, rendering them essentially useless. I want to return these tests and have the ******** payment reversed. I have made two phone called to the phone number listed for Cipher Global given on my EOB from ********, on 12 April and 14 April during business hours. Neither call was answered.
