****** ******** ******************************************** Email: ********************* ************** Date: May 27, 2025 Better Business Bureau Serving ******* and **************************************************************************************************************************** Subject: Formal Complaint Against Midwest *********** Clinic Account #****** Date of Service: January 20, 2025 Dear Better Business Bureau,I am writing to file a formal complaint against Midwest *********** Clinic, located at ***********************************************, regarding a serious and unresolved billing issue associated with my account number *********** than five months ago, Midwest *********** Clinic received full payment for services rendered from my insurance provider, *****. In addition to the insurance reimbursement, they also received an overpayment from me directly. Despite multiple attempts to address this with the clinics billing department, I have yet to receive a refund for the overpayment, which is in excess of $200.00.The clinic has consistently failed to take responsibility or offer a clear explanation. Instead, they have continuously delayed action, citing various internal issues or providing vague responses that have only prolonged the resolution of this matter. I am extremely dissatisfied with the lack of professionalism, accountability, and transparency displayed throughout this process.I am requesting the assistance of the Better Business Bureau in facilitating a resolution to this matter. Specifically, I am seeking the immediate refund of the overpaid amount.Thank you for your attention to this matter. I look forward to a prompt and fair resolution.Sincerely,****** ********, ***Business Response

I am rejecting this response for the following reason: I have communicated with multiple representatives from the Midwest *********** billing department regarding my refund. On several occasions, I have been told varying timelinesinitially 30 days, then 90 days, and subsequently an additional 90 days. This pattern of delay gives the impression that my refund is being intentionally withheld, which I find unacceptable and unfair as a consumer.
The original date of service was January 30, 2025, and it has now been six months without resolution. I am respectfully requesting that a refund be issued promptly.
Sincerely,
****** ********Business Response

Certainly! Heres a revised version that emphasizes your repeated, unsuccessful attempts to obtain your refund or overpayment:
Complaint: 23380768
I am rejecting this response because, despite my repeated efforts to obtain my refund/overpayment, I continue to be met with the same delays and lack of resolution. Most recently, I spoke with a representative named ***** on June 9, 2025, at approximately 10:00 AM, using the number provided for the Customer Advocate. Once again, I was told that the process would take an additional 90 days.
This is not the first time Ive been given this timeline. I have heard the same explanation on multiple occasions, with no meaningful progress or results. It is extremely frustrating to be caught in a cycle of promises without action. I respectfully request that this matter be escalated and resolved without further unnecessary delay.
Sincerely,
****** ********Initial Complaint

I am sending an officecomplainton behalf of my 7 year old child, Ariyah Battle, who went to urgent care on 4/20/25 at Midwest Express Clinic at ************************************************************. I brought my child in due to excessive vomiting(every ***** min )that started at 4am until about 8am which contained only mucus. Upon arrival, I provided them with the following complaints: congestion, sneezing with globs of mucus hanging from her nose, and severe vomiting that contained mucus, persistent coughing. I told intake my child has a history of ************ uses ********* when she has respiratoryillness. The Nurse PractitionerAshley ***** failed to provide a basic comprehensiveexam. She did not look in her ears nor did she provide any information on how her lungs sounded. She said she has a virus and only gave ****** for vomiting and a cough suppressant. My child had not eaten the entire day, and could not stop coughing. I took her to see her pediatrician Dr. ***** located in ************** at Specialty Physiciansto get an accurate diagnosis which was an ear infection and lung tightness the very next day at noon. She finally was prescribed antibiotics. My child actually ended up needing two additionalappointments with her pediatrician because she was so sick and the first round of antibiotics her body was not responding too.The nurse practitioner ****** ***** at Midwest failed to provide use with discharge papers as well.I called the patient advocate to complain and no one left a message indicating they receivedmy message. The billing told me on 4/21 I could get a refund of my $30 copay since I had to take her to her doctor the next day to get a correct diagnosis and antibiotics. I was told to call back5-7 business days later. Upon calling back, they gave me a hard time by saying it was under review with the clinic. I had to call twice for them to say I'm not owed a refund of my copay because the chief complaint was only a cold and cough which was untrue.Business Response

I am rejecting this response because:the issue extends beyond a refund. My concern involves the quality of care and professionalism during a child examination and accuracy in diagnoses. The care my child received was subpar and potentially harmful if I did not seek a second opinion from her primary physician the following day. I acknowledge the refund and apology, however, it does not adequately address the seriousness of the concerns raised-particularly of clinical standards and accountability. I only received a refund due to escalating this issue which undermined my trust in services provided. I am continuing my care through ***** to ensure a full review of professional conduct. Medical professionals have a duty of care that was evidently not provided to my child.
Sincerely,
Alexandria BattleInitial Complaint

Midwest Express Clinic requires a card on file prior to receiving services. Bills are mailed and if unpaid 21 days after statement date, the card on file will be charged. My card was charged before he 21 days and I didnt receive a bill before my card was charged. The bill arrived 12 days after the statement date. My card was charged 11 days after the statement date. Billing and collection practices are unfair and deceptive and the clinic did not follow their own policies and procedures. My bill did not arrive timely to substantiate charges and the clinic charges my card at a time earlier then dictated by policy and procedures.Business Response

Hello, After I left my initial complaint, I was called by the customer advocate center and they refunded money that was taken out of my account without my permission. I was told my card number was going to be deleted as well. A day later, they take double the amount out of my account! My bill was $72, they took it out of my account without my authorization, refunded me $72, then a day later took out $144! I am furious with this and will be calling my bank. This is such fraudulent behavior. I have never had this issue at any urgent care.Business Response

I visited the ****, ******* location on 2/13/2025 with my daughter. We are self pay and the staff charged me $125. When I was leaving, they informed me they accidentally put my payment on the River North store and they need my card again to charge the $125 to ****. They stated they'd refund the other $125 for the incorrect error on their part. Since then, I have not been issued a refund. I've been on hold with the billing department for hours each day with no answer. I tried to do the call back and message option and never received a return call. I also did an online complaint and still have heard nothing. I called the **** office and they said they don't see the refund, but there's nothing they can do - I have to call billing. I confirmed I have the correct phone number.Business Response
