    Date of transaction: On or around 5/17/2023, I paid The Growth Center $150 cash for an appointment with ******** ****** The business stated ******** was "working on getting her license" but is seeing patients and committed/able to prescribe: my adhd and anti-depressant medications.
    Nature of dispute, 2 things:
    #1- I was advised by the pharmacy that ******** ***** is not contracted with my insurance or any insurance so I would be have to be self pay or find a different physician. This was never explained by the Growth Center. The pharmacy also advised that ******** is not licensed, therefore someone else in her office who was legitimately licensed would be required to prescribe/ re-write my C2 rx because as is, not valid. #2- *** ******** the medication coordinator, presumably competent at her job, refused to provide the necessary steps my insurance required for a prior authorization once Dean O'Brian, a legitimate licensed Physician that re-wrote-re-sent in my rxs or adhd/depression to my pharmacy at the time. I spoke with *** a number of times and she made excuses as to why she was not going to complete the appropriate prior authorization and submit to my insurance company. *** stated "Oh I asked our drug rep and they stated wait 5 more days and then you can get your rxs filled no problem". That's not what my insurance company stated would be required in order for my rxs to be filled and the insurance pay for it. *** refused to help me in any kind of way as my most previous rx had been tampered with and destroyed and I was without. I told *** and ******** that, they both knew that, but I guess didn't give a damn and was deceptive in taking my money for payment without providing acceptable and integrity of service. Because ***, the claimed "Medication Coordinator" who represents the Growth Center REFUSED to complete the prior-authorization forms I had to go find another provider and pay more money out of pocket that I did not have to spare for the exact same thing.
