      I received services from Heffingtons South on 3/13/23 for an injury I got at work. I disclosed this appointment would be part of a workers comp case and I should not be receiving a bill, the company should. I have verified with the company that the bill has been added to my workers comp case, but Heffingtons keeps sending me bills for this service. This is not something I should, or will be paying for as it was in my workers comp case.

      To Whom it may concern:

      ******* ****** was seen by Dr. ***** ********** at our **** ** ********* ******** on 03/13/2023. At that time she signed an Acknowledgement of Assignment of Insurance Benefits and Privacy

      Policy. This forms states that we will file on behalf of a patient, for the benefits available on their behalf with their third party payer. It further states that if the third party payer fails to pay us, the patient is responsible for the services billed. At the time of service Ms. ****** signed this form. It attached to this letter.

      However, as a good faith measure, we have written off the charges as bad debt and have marked her account no further service.

      No future bills or correspondence will be forthright from our establishment to Ms. ******.

      Toward the end of April 26, I went here to get new glasses. I told the rep that the first couple pairs of glasses I didn't like because they were too big for my face. I asked her for a smaller sized frame. So she said she will see if they have any. She told me to try these. I told her that's better. They made the glasses and I went back 3-4 days to pick them up. I told her they were still too big. I tried them at home and they were still too big. So I took them back and she said there are no refunds. They measured them and said they could put them in another pair of glasses. I went back a week or so later, they were the same glasses just tightened up on the ear pieces. My face is narrow and I need a smaller pair of glasses. I need a size 48. I don't think they have a pair in the place that would fit me. I shouldn't have to tell them what sized glasses I need.  There were 2 prices on every pair of frames and I thought our price (with insurance) was the cheaper one.  The top price was for insurance $315 and the bottom price was $200 without insurance.  I want my money back.

      We have reached out to the customer and informed her to bring the eyeglasses back and we will refund all of her charges.  She brought them back this morning and we have given her a full refund. She is very satisfied with her complete refund of $511.79 - this amount is the full amount she paid for her eyeglasses.  This matter is resolved with the customer. Thank you.
