      My husband and I went into this urgent care as we were in an auto accident. The dr failed to write the correct information down that we stated to him. When we went back to get the visit notes, we were provided with other patients visit notes. Upon getting the correct visit notes we noticed that the Dr mixed our visit up with someone elses. The nurse put my husband and I in separate rooms because she indicated Dr. ***** has issues keeping things straight. The notes mention nothing about my husbands chipped tooth. They have in the notes for both of us that we were not wearing seatbelts which we always wear a seat belt and were never even asked this question by anyone at the clinic. Both my husband ****** & I were having pain in various areas. Someone on my report it states " Throat Culture showed no bacterial growth ain to the left hip. I NEVER had a throat culture as my throat was not a complaint of mine. Why would anyone need a throat culture for a pain in hip?? The report is so convoluted with errors its pathetic. I refuse to pay for my husband or my visit after this debacle. In addition, we are having to be seen elsewhere so we can get an accurate diagnosis.

      I received your notice of a complaint that was submitted on 1/8/2023 and assigned # *********  Ms **** ******* had complained that after she and her husband were seen that the medical record did not reflect accurate information and they had to be seen at another facility.  Ms. ******* had also reached out to me, I had the patient records corrected.  We had provided the corrected/updated records, and had as a courtesy written off the charges for those visits.  This was completed on 1/13/2023, so I believe that this was settled to the patient’s satisfaction according to your listed desired settlement.
