      I contacted The Assurity life insurance. ***** after my mother passed away and she had assumed I would be able to receive $******* in insurance money.

      The person told me she had a regular insurance policy ., and I SAID " Are you sure ? "

      and he said " Yes", she had a regular insurance policy..

      My sliding glass door is broken so I needed another one. .
      I got a loan from my aunt for $***** because of this because I BELIEVED I could pay it right back with this money ,

      and I also opened up a credit card as well.

      Spent around $*** on basic necessities ***************************************************

      ********************** I know I cannot afford these extra payments otherwise..

      and I would NOT EVER DO THIS ....

      UNLESS I thought I could pay the money RIGHT back !
      .Assurity never contacted me..
      nobody told me anything ..
      So, I called back, and they said oh we're sorry we closed your account it was accidental insurance ..
      So I was misled to think she had a regular ins.

      and THAT IS why I took the money out. And don't like spending money that much but I got gifts for my family and stuff ..because I had the extra money ..( well I thought)

      . I need them to pay this money back to me
      I NEED **** TO PAY BACK THIS DEBT NOW BEFORE MY CREDITS RUINED .

      ***************** I can't do this with my other insurance and credit card bill ,now that I have been taking on more bills now because my mom helped me with the mortgage before Or with heat. Because we were buying this house ,before she died .

      It's not acceptable that someone would say that I would have that money and then.. the agent says , "oops I didn't read the full report and I told you wrong ."

      And they said sorry for your husband's death on the FORM also .
      but it was my mom that died..

      I need the **** to pay back the loans .

      We are writing in reference to the above case, which involves a claim filed under an Accident Only Term Life Insurance policy issued December 10, 2009, by Assurity Life Insurance Company.

      At Assurity Life Insurance Company and we take seriously our responsibilities to regulators, auditors, and most importantly, our policyholders to treat them fairly and consistently, paying all benefits due based on the terms of the contract held by each policyholder.

      In issuing this policy, the insured was required to sign a Acci-Flex DISCLOSURE STATEMENT which states, "The face amount of this policy will only be paid to the named beneficiary if the insured dies as a direct result of an accidental bodily injury. An accidental death is one which results directly from an accidental bodily injury suffered while the policy is in force and is independent of all other causes. Accidental death must occur within 90 days of the date of the insured's accidental bodily injury, and prior to the policy anniversary nearest the insured's 75th birthday, and while the policy is in force. No benefits are payable if the insured's death results from a cause other than an accidental bodily injury. I acknowledge the receipt of this Acci-Flex Disclosure Statement." This disclosure statement was signed by the insured and agent on December 9, 2009.

      As outlined in the Payment of Proceeds section of the Accident Only Term Life Insurance policy, it states, "If the Insured dies as a direct result of an accidental bodily injury (Accidental Death) while this Policy is in force, We will pay this Policy's Proceeds to the Beneficiary. We will require surrender of the Policy prior to payment. Payment will be made when We receive proof of death and the Policy at Our Home Office. Accidental Death is death that results directly and independently of all other causes from an accidental bodily injury suffered while this Policy is in force. Accidental Death must occur
      within 90 days of the date of the Insured's accidental bodily injury; and
      prior to the Policy Anniversary nearest the Insured's 75th birthday; and
      while this Policy is in force."
      The information obtained from the death certificate indicates the cause of death as ************************************************************************************************************************************************************. As the manner of death was due to natural causes and not an accidental injury, our position remains that no benefits are payable in accord with the policy language.

      We do acknowledge that one of our correspondence did state husband rather than mother as it should have stated. While we sincerely apologize for the error in the correspondence, we do not find that it impacted the eligibility determination under the policy.
