*********** for the amount of $594.00. I have some questions about how the determination was made.
The breakdown of expenses did.not include any of the billfrom Kansas University Medical Center, which was where the final treatment was performed. According to the "Schedule of Operations you sent, you did not include anything for:
Operation through abdominal wall for removal of organs in abdominal and pelvic cavities Resection of the small bowel
lleostomy
Complete cystectomy with ureteral transplant Ureterostomy

This company has charged me $62 for the last several months. I have had several family members ill and had deaths in the family so I was not diligent about checking thoroughly. Big mistake! I called this company and they claimed that I had a life insurance policy and even gave me, some random person‘s name with a policy number. I’ve never taken a policy out or even use this company. This is a complete scam! I contacted my bank to let them know as soon as I became aware of how often they were doing it.Business Response

I called the company to cancel the policy . They refused . Finally they asked for me to send cancellation on a piece of paper signed. I sent the letter and signed. They recently sent a letter stating t(at I will continue to be billed . I would like my fees refunded. They have added fees and refused to cancel the policy.Business Response

My sister and I inherited a WN annuity from our mother who died in Jan 2023. Our mother made annual withdrawals from that annuity for several years before her death. Those withdrawals cleared the annuity of most of the taxable interest income that accumulated over the years. When WN prepared the 1099R they failed to recognize those withdrawals. So the 1099R which they issued overstated my taxable income and that of my sister by over ten thousand dollars. I notified in writing the third party administrator in Nashville TN and also the home office in Lincoln NE. Meanwhile our tax returns are due in about a month. I am rejecting this response because: they have not responded to me about annuity policy ******** which my sister and I inherited from our mother.. They have in the past issued 3 forms 1099R for this annuity none of which are correct. I have explained to them what the correct numbers should be and how to calculate them

After calling Washington National, I am being informed I made a policy account with an Agent named Sydney ******* and started paying for life insurance at the start of the year 2023 for $***** per month.
I have no recollection of making any sort of appointment or call and have no evidence in my emails or mail of any correspondence with Washington National or Sydney *******, apart from the document they sent me to cancel my policy.
I would like a refund of any amount that I have been charged towards Washington National.Business Response

I file a claim after I had surgery on my hand.When I bought this hospital indemnity I was told if I was seen in the hospital and being in a bed for a couple of hours that I will be paid ****$ one time per year. So I had to have surgery I was in the hospital fro 7am to around 5pm.Now I fill the claim they approve me ***$ not the ****$ because I wasn't admitted in the hospital.I told the lady who spoke to me what I was told when I bought this insurance her answer was for the person to sell the insurance they will say anything .Told me did you read the policy I said no because I trust in his word. For being laid too and being robbed , I want at least half of all the money I paid to them back ASAP. Hope they stop selling fake insurance.Customer Answer
