      My 78 year old mother spent more than 21 days at the Autumn *************** facility following her recent hip surgery. I was listed exclusively as her next of kin/ primary point of contact, yet this finance company has billed me as a responsible party for my mothers bill. Attached, you will note the bill reads: Resident "***** *******", yet it is billed and mailed to me, "******* *******". I have called ************ to dispute the bill and even sent emails, yet the associates who answer are less than pleasant, always annoyed and appear to be unaware of why I should not be billed, despite me sharing with them that I WAS NOT AT ANY TIME A PATIENT OF THE FACILITY! I was given the name ******, ***** and now a **** *********, not one answers, responds or returns my messages!Please help me dispute this matter and recommend the company reissue the bill, made out to my mother, the actual patient at the nursing home/ rehab facility! Earlier today, May 29, I spoke with a manager at the ******* location, and she informed me that this finance company does this regularly despite the knowledge that the people they are seeking payments from, are not the responsible party! They seem to bill whomever is listed as next of kin/ point of contact.

      My mother was a a patient there last fall of 2024. I'm receiving a bill for her care for the dates from 10/2 - 10/27 I've explained to them on numerous occasions that this has to be billed to her medical insurance at the time she was on ********. The facility removed her from her United healthcare ******** plan to extend her stay I should not be receiving a bill for her stay for $5,5508. I also never signed any waiver making me her financial proxy now I'm getting a letter stating that this matter will be going to a collection agency. I'm getting no resolve from the facility after leaving phone calls with numerous people.

      My mother was admitted to the ******** Nursing facility in Valatia, ** and I had filled out the ******** application and handed it in within 3 days. The nursing home failed to file the application within the time frame of full ******** coverage, leaving a balance for the first month of her stay. I had paid $5,000 from my mother's account on her first day of admission. They not only are trying to have me pay the balance of $6,000 from the first month, but has kept the $5,000 paid to them. I am not responsible for any of this, especially due to thier negligence in failing to submit the application in time. My mother had passed away last February of 2022, and they are still contacting me and sending bills.

      On January 12, 2023, Future Care Consultants **************** ********, ********** **************, took it upon themselves to access my mother's checking account without prior authorization or notification from her daughter and ***, ***********************. ************************* was a resident in the rehabilitation unit of *************************** (ALAW), *********, ** appx. November 2022-January 4, 2023. During that time it was discussed but NOT followed through that she would become a long term care resident (LTC); however, ****'s doctor declared her able to consent for care and she allowed to be discharge herself from the facility. During this stay it was agreed upon the her intake paperwork for LTC would be immediately deleted and rescinded (not be processed for use). On/about January 10, 2023, my mother ************************* was returned to ALAW for rehabilitation services and eventual transition to LTC. I met with the treatment team and social worker (******) via conference call on 1/11/23, with the understanding that she would NOT immediately become a LTC resident and no LTC intake paperwork would be needed at this time. On January 12, 23, Future Care Consultants **************** ********, ********** made an authorized transaction and did not notify the *** of $2,100.00. ************************* has a ******** and ********* so there was absolutely no reason for them to access her account from the intake paperwork from her previous visit, November 2022-January 4, 2023. Future ******************** should not have retained her information if she was no longer a resident there and they did not have permission to access this information to process any payments without contacting the *** and providing an explanation or seeking updated permissions.I reached out to Future Care **************** spoke with someone by the name of *****************, who said they would not return the money to her account unless agreed to a care plan moving forward. The transaction was illegal and unauthorized.
