******************** is handling my *** claim for me. Im out of work suffering from several issues and ******************** use different tactics to discourage claims. My *** payments started 6/7 so the first month I received my payment 6/4. For July my date was moved until 7/17 to reflex my scheduled return to work date. They constantly push your payment date further back. Did the same thing with my *** claim as well. My case manager **************** and her supervisor ***************** is always non responsive. When you either put in a call back request or ask for your call to be escalated it take more than the 48 hours for someone to call me back. I have called ******************** since 7/5 and I still havent received a call back from my worker ****** or her supervisor *****. This company is beyond evil and heartless. My sickness cause me to not be able to work and since Ive been waiting for them to process my payments I have been taken to court by my landlord and had utilities cut off. I already write my checks for my bills because I was told on Friday that my payment will be released today 7/8. When I didnt see the payment this morning I called and was told that my payment wont be sent out until the 17th. This company is purposely taken weeks out of my check because when ever your pay date is you will receive another check 4 weeks later. My payments are supposed to be the first week of every month not the 17th. This added stress isnt good for my mental health and its really sad that a company can legally do this. I will be contacting a lawyer. Its not up to ******************** when I showed go back to work, its up to me and my doctor. ******************** purposely try to make it difficult to get paid to force us to go back to work. Dealing with this company has been a disaster, its no case monitoring by upper management because I have requested supervisors, *** requested another case manager, I have requested my calls be returned and its all being ignored. I have been paying for Short-Term Disability Insurance with Guardian Insurance Company. I was involved in a car accident. The other party was at fault. I am on FMLA. I filled for Short-Term disability. My attorney has explained, to Guardian, that the car insurance policy probably is only for $30,000.00. My medical expenses has exhausted that. Guardian will not pay my disability claim stating, it should be included it the amount billed to the car insurance company. Law suits can take years. I paid for Short-term insurance out of my pocket. It should cover me if I am disabled.Business Response

On Jan 29 2024 I had a dental exam check up and cleaning which was supposed to have been completely covered by any dentist I went to as it was preventive care. I confirmed with both my dentist and marketplace whom i purchased this insurance through ahead of time that it would be completely covered through Guardian insurance who I had paid for my insurance coverage. Then last week I got a notice from Guardian they werent paying it and I would owe my dentist $155. If they hadnt lied in my coverage I would have never bought insurance through them as the amount Ive paid them since ******* is more than the amount of the dental bill.Business Response

I have dental insurance through Guardian. This company is a total rip-off. They are not paying for claims that were submitted in ********* wife's name. Their website is not functional. My ******** chose our primary care dentists a while back. Their website does not list the correct primary care dentists that my ******** chose had previously chosen. Their ************* customer ********************** stinks. It's impossible to speak with a customer ********************** supervisor when you call Guardian. And I'm still not able to look at past claims on Guardian Dental's website. Whenever I try to search through my past claims, I get this message:
"Your document should be available shortly. We apologize for the inconvenience. Please try again in a few minutes."
A company's website that won't even let me search through past claims is unacceptable.
Guardian Dental still has *******'s primary care dentist listed incorrectly.
Therefore, my BBB complaint has not been resolved.

My wife *************************** and I are having a problem with Guardian Dental Insurance.First lets talk about my issue. I had my dentist and a Periodontist explain that I needed an extraction and an implant, I was pre approved by Guardian, then after the procedure they started coming up with reasons not to pay. What is interesting is that their internal doctor did not know I had been pre-approved. We received multiple notices saying the proceedure was not needed and they would not pay. After two months of phone calls I threated legal action for pre approving me and then denying payment. According to thier customer ******************** department they did not have the pre approval linked to my claim. Suddenly it was paid. Sound a little fishy? It was, just look at all the complaints on the Better Business Bureau:Guardian Life Insurance Co. of America | Complaints Better Business Bureau ************************************** profile insurance-companies I'm paying for dental insurance and am getting nothing for it ... Guardian is fraudulently collecting premiums from employers but not paying out claims So my wife ********, same dentists as myself, gets a crown due to a cracked tooth and once again, their internal doctor is denying it was needed. Our dentists have very good reputations. They conducted all the correct tests for Cracked Tooth Syndrome and said she needed the crown. Once again it was denied. We appealed and it was denied again. According to our dentist and our Perodontist they are playing games. We have all the pictures and descriptions of the tests that were completed for ******************************* Cracked Tooth Syndrome that we uploaded to you. The dentist and Perodistist are willing to act on our behalf.Please help my wife resolve this issue with her Crown. I really think Guardian needs to be investigated based on all the complaints. I have dental insurance with the company I had work done with dentist last year and this year i put a claims for reimbursement for both years . the company is trying to exhaust me so they don"t pay me they have put both cases on pending with many questions that are on the claim papers. i gave them all the information lately they keep asking if the name of my Dentist is true then if the date of the claim is true every time a different verification that is ridicules.this company is trying not to pay the people who insure thru them I am owed two payment each of $5000 total of $10,000 i paid out of pocket $35000 plusBusiness Response

Guardian was given proof of cobra benefits for March and April of 2024. They refused to pay even though the dentist has checked eligibility on the dates of service before providing services. The plan administrator even called guardian dental while we were also on the line to inform them that we were covered. We have proof of coverage and guardian refuses to provide a way to send them the proof. Its an endless circle. The supervisors never call us back and we cant get in touch with anyone other than a foreign call center who isnt allowed to do or say anything. The dentist also complained saying this is a common problem with guardian. Im guessing you have several complaints filed by others.Business Response

I signed up for a 10 yr term plan with Guardian Life Insurance. I was told repeatedly that it would be $40/month. They keep charging me $472/month. They are legit robbing me. I have not authorized this amount and it's actually insane considering I'm 36 years old. I do not want to do business with such a shady company. Guardian has provided terrible services and communication for months regarding disability claims. I have reached out to the company directly as well as filed multiple complaints with no follow through or clarity. If they refuse to pay then they should deny the claim not drag it out for months. I am writing to file a formal complaint against Guardian Dental Insurance regarding a misleading information issue that has caused me significant inconvenience and financial loss.On February 25, 2024, I purchased a dental insurance policy from Guardian Dental Insurance (Policy #: ********, Member ID#: ***********. I was explicitly informed by the Guardian representative that the policy would cover implant procedures, which was the primary reason for my enrollment.However, upon submitting a claim for the implant procedure, I was informed that the coverage only applies if the implants are placed at the same time as extractions. Since I did not have any teeth at the time of the implant procedure, my claim was denied. This specific condition was not communicated to me when I purchased the policy.Due to this condition, the dental insurance policy offers no applicable benefits to me, as I do not require other services such as cleanings. Consequently, I have paid $226.47 in premiums for three months ($75.49 per month) without receiving any benefits.Despite reaching out to Guardian Dental Insurance to explain the situation and request a refund of the premiums paid, I have not received a satisfactory resolution. Therefore, I am seeking assistance from the Better Business Bureau to address this issue.The resolution I am seeking is a full refund of the $226.47 paid in premiums ($75.49 each for March, April and May). I believe that Guardian Dental Insurance has engaged in misleading sales practices by not fully disclosing critical coverage conditions. This has resulted in a financial loss for me and a policy that provides no value.I kindly request that the BBB investigate this matter and assist me in obtaining the refund I am entitled to.
