- Additional Info:Our BBB began receiving phone calls and inquiries on this company on 1.22.07. Callers indicated they had received a notice advising them that their personally identifiable information may be at risk. Our BBB called the Dallas office of Concentra and spoke with a company representative who confirmed that the mailing is real. The letter was sent by Concentra on behalf of its' client Nationwide Insurance. According to the company, a satellite office of Concentra located in Massachusetts was broken into and back up tapes along with cash, and laptops were stolen. The tapes contained encrypted patient information. The letter asked consumers who wanted more information to call their offices at 1 (888) 858-4935, Monday - Friday, 8am to 6pm Eastern Standard Time. The letter also provided detailed instructions no how to put a fraud alert on your credit report with the three credit reporting agencies.
