      I received an unjustified bill for $193.90 for 2 visits from this office. I went to this office for the first time on 8/29/24 for an annual check-up. This is "preventive care" and covered by insurance. I specifically stated when I called to make the appointment that this is an annual check-up, and the scheduler said she would schedule a check-up. When I arrived, they gave me a new patient form to fill out which asked what medications I was currently taking. I added to the form that I currently take Fluoxetine (for over 20 years) under the list of medications on the form. At the end of the visit the doctor asked if I needed a refill on Fluoxetine and I answered yes, as I do at every check-up, no other discussion about it. I just received an unjustified bill for $109.73 for my check-up- they did not even do bloodwork and said I'd have to schedule that on another day.

      11/9/24: I scheduled another appointment at this office for a swollen toe (I had not received the FIRST bill and did not know they had made an unjustified charge). The PA advised she would treat me for a toe infection with an antibiotic (Doxycycline). I told her I felt this was damage to my foot and toe and may need an x-ray but she said "we will use process of elimination". I advised I have Ulcerative Colitis and IBS and get sick from antibiotics, but she said I should be fine. I did get sick from this and they billed me for $79.13. When my new insurance kicked in I went to a podiatrist, who diagnosed nerve damage to my foot.

      2/13/25: Spoke to office mgr Bonnie Rozema. Advised I was given a prescription meds for a mis-diagnosis, and asked that she waive the 11/6/24 charge, and bill the 8/29/24 charge as a check-up. She asked when and what podiatrist I saw and I confirmed, the she said "well, I'm not changing anything." I should not have to pay these unjustified charges- I live alone, am on a tight budget at $34k a year and a single income household, and this is a financial hardship.

      Patient presented as a New Patient on 8/29/24 and she was
      scheduled as such.  She had complaints/chronic
      conditions that were addressed at that visit. 
      Medication was prescribed.  The
      visit was coded appropriately with a New Patient Evaluation and Management
      code.
      Patient was scheduled as a same day appointment on 11/6/24
      for swollen toe/pain.  Patient was given
      a differential diagnosis at that time as she was “not showing classic symptoms
      of any particular diagnosis.”  The
      patient was referred to a podiatrist at that visit.  Labs were ordered and obtained that same day.
      I received a message from our Billing Department saying the
      patient was disputing the charges on 2/12/25. 
      I called and spoke with the patient on 2/13/25.  She reported that she had seen a podiatrist
      and we mis-diagnosed her.  I tried to explain
      to her the difference between a physical (which I can only assume is what she
      means by “check up”) and a regular office visit.  I also reviewed the note from her visit with
      regard to her foot and explained to her how our provider did her due diligence
      with regard to her care and referral. 
      Our conversation ended abruptly because she had to get back to
      work.  I never said, “I’m not changing
      anything,” but I did respond to our Billers to let them know that she is, in
      fact responsible for both dates of service. 
      I followed up with the podiatrist we referred her to and she
      never made an appointment with them.  I
      did speak with someone at the podiatrist’s office she referenced in our call
      and she was scheduled, but hadn’t seen them yet.  This was three months after her initial visit
      with us regarding her foot.  Of note,
      patient has since reached out to us as recently as 3/18/25 for a referral for
      another medical complaint and we obliged.

      I am rejecting this response:

      I have been checking for this response daily, but just found it this morning in junk mail or I would have responded on March 24th when it was received. There are several statements of falsified information made by Bonnie Rozema in her response. Again, when I called to schedule the appointment I scheduled it as an annual preventive check-up, which is no cost to the patient as it is covered by my Cigna insurance. I am requesting that this doctor's office submit a recorded copy of the phone call where I scheduled the appointment as an Annual Preventive Care check-up appointment ONLY. As I mentioned previously I completed the required patient information where I was asked to confirm what medications I was currently taking, which was Fluoxetine. At the end of the check-up the doctor asked if I would like a refill of this and I said yes. NO OTHER COMPLAINTS were mentioned by me I only answered the questions that the doctor asked ME. If this office has the intention of changing the type of visit (to boost company profits) then they are obligated to inform the patient they will receive an overwhelming bill at that time when they call to schedule a no charge preventive care appointment. I have seen several other doctors for the first time for an annual preventive care check-up and have never been charged or received any bill. I saw other similar complaints on their patient reviews. Secondly, during my conversation with Bonnie she asked me to confirm what podiatrist I saw, the date and appointment time, which I DID confirm to her immediately when she asked. Another falsified statement from her- that I did not see a podiatrist. I have attached the receipt from Physicians Foot Care for BBB to view, where I was diagnosed with a Neuroma (nerve damage) and am currently being treated for this by them. I chose my own specialist who is in my current insurance network. My visit was on 2/10/25 with the podiatrist and I did not speak with Bonnie until 2/13/25 so if she called the podiatrist that I saw, they would not have stated that I did not see them, more falsified information. She DID also state after I confirmed the podiatrist visit that she will NOT be changing any of the billing so I told her I would not argue with her at that point- another falsified statement. Hopefully this is also recorded so she can submit this conversation to you as well. She also failed to mention to you that the PA Stephanie Xavier during the 11/6/24 appointment for the foot problem instructed me to take a prescription of Doxycycline for 1 week, due to her misdiagnosis of Celulitis. I told her I have Ulcerative Colitis and generally do not take antibiotics but she said it should be fine. I did have digestive symptoms during and after the prescription. I have also attached this prescription for BBB to view that I was instructed to take. Thirdly, when I recently contacted their office on 3/18/25 to request a referral to see a cardiologist, they did NOT oblige, another falsified statement of many. The front desk stated they could not give me a referral and when I asked why she disconnected the line, and I never heard back from anyone. There was also mention of my waiting 3 months to see a specialist- I was waiting for my new insurance, which started on Jan. 1st, as it covers visits by co-pay to see a specialist, My Cigna insurance did not cover this and visits were out of pocket. I will not pay even 1 cent on either of these bills- EVER, for the misdiagnosis due to incompetency- where I was given a prescription for no reason...this charge should be fully waived, and I will not pay (EVER) for this falsified bill created for my preventive care check-up, for the purposes of the business's profit. If they want to be paid for the preventive care check-up of 8/29/24, they will need to rebill correctly as it should have been originally- to receive payment from the insurance company.

      Please review the attached medical record for my recent visit to a podiatrist where I was properly diagnosed with a Neuroma. Due to being misdiagnosed and treated with antibiotics for Celulitis at Liberty Doctors I would like for the charge for the visit to be fully waived. I have already requested this directly during a phone call to the office mgr. but she refused. I will not pay this bill in which I was charged for incompetent medical treatment.

      I do not see any attachments with regard to her visit with podiatrist.  If HIPAA allows, I would be happy to forward our notes from the patient's visit.  Other than that, I stand with my previous statement.
