On January 25, 2024 purchased expired health food drink at ***********. January 29, 2024 received e-mail communication from a claim representative ***************** with Sedgwick (insurance company for Dollar Tree) to forward to her photos of expired drink and receipt and send all medical paperwork relating to incident. On February 7, 2024 I responded to Ms. ****** notifying her that I would send over the requested materials as soon as I received all my medical records hopefully by ********. On, 2-13-2024 I email ****************** to inform her that there would be a delay for 2 weeks to receive all the records that I planned to send over to her. On 2-15-2024, I went ahead sent over whatever paperwork I had already, however, Ms. ****** responded to my email and stated that she was going to be out of town on vacation and returning the last week of February. Therefore, On March 18, ************************************************************************************************* ******. Unfortunately the representative did not contact me at the end of the month of February as she had stated to me. However she sent an email on 3-19-2024 with an offer of $250 that I was unaware of due to me being out of town during that time. Then on 4-11-2024 Ms. ****** sent a second email asking if I wanted to accept the money to settle the claim and that the file would be closed at the end of the month on 4-30-2024 which again I was unaware of because again I was out of town and not available to have access to personal email. Therefore, after returning from out of town in May and upon checking my email I realize the last email from Ms. ****** was sent to me on 4-11-2024 and the file closed on 4-30-2024. I immediately tried calling Ms. ****** leaving several message and sending emails to try and speak with her about my absence and ignorance to the closure of the claim. ****************** refuses to return my phone calls or respond to my emails since May, June and July regarding the claim.Initial Complaint

Sedgwick is handling a bodily injury claim due to my injury at Lowes store in ******, ***I have been trying to connect, through the use of the telephone and emails,Wwith a Sedgwick employee who can discuss the settlement of my claim. Despite spending hours and hours on the phone or on the Internet, I have not been successful, unsecuring any cooperation that would allow me to speak with the person assigned to my claim. I do have a claim number which was passed on to Sedgwick. I am an injured worker who's employer uses Sedgwick to manage claims. For almost 3yrs, Sedgwick has continued to pass my claim from adjuster to adjuster. I am completely ignored. Each adjuster that I attempt to contact has either quit the company or cannot be reached. I believe Sedgwick has juggled my case between 7 different adjusters now. I am simply awaiting documents that would close my file and render my benefits that I am owed. Sedgwick is clearly using unprofessional tactics to avoid paying out the benefits owed to employees and consumers. On June 19th ATT came out to fix the fiber issue our entire neighborhood was experiencing. The service men used a large excavator to dig up my front yard as seen in the picture. In the process they the cracked my driveway, and broke pipes and wires for my lawn sprinkler/irrigation system.I started a claim and was referred to this Sedgwick company. I have called them multiple time but all I ever get is a voicemail. I have registered online and used their communication portal but no one has responded in weeks. I work for a company that uses this company for medical leaves. I started a leave back in April which my doctor wrote me out for 30 days specifically on paperwork and they only approved half of the days and the stated the remainder days was not enough to cover even though the medical was able to support half of the days with the same medial condition and notes. Then they stated I needed to see another level of medical for help in which all appointments was booked out so far I went to see another specialist similar until I can get booked for the specific specialist they needed me to see. I provided all documents Im currently going through an appeal process which has been the worst no one ever calls they only text and send letters. They stated they sent an email but was spelling it incorrectly which made the process longer. I sent over eviction paperwork I cc the ceo in emails. Ive been down so bad I went out on another leave and been providing paperwork to get paid for that leave only to find out weeks later they combined the leave with the last leave which already been denied so now its included in a appeal that takes weeks when I havent been paid in months. I have a 4 year old and literally a few days from being homeless and its sad. I just want help from someone that knows that they are doing, I want verbal communication only and I want to get paid.Initial Complaint

My husbands company transferred him and Im the process moved us. They took care of the whole move from start to finish. They came to our old address packed everything up and, loaded it into a truck. A trucking company of their choice and stored our belongings at a storage facility of their choice, which we had no access to. When we moved to our new address several months later we received our items. They were supposed to mark the boxes for the appropriate rooms. We had items from our garage put in some of our rooms that they loaded into our house. After unpacking we found a lot of damaged and broken things. We also had missing items. Our things were thrown in boxes that had toilet brushes and garbage cans put in with our clothing. We had two surround sounds and many other electronics that were not wrapped up and protected, all in different boxes. Our speakers were thrown in with hammers, s**** drivers . screws ect. All my sons PlayStations, Wii U, switch cords and remotes were not put together when they were that way to begin with. We asked for our beds to be stored a certain way. They took pictures of them when they were first loaded and stored. When they were loaded to be taken to us due to delays they were in the truck for days not loaded correctly and were seen them being unloaded. We filing claims immediately and were told the company we signed on with was not handling our claim because it was within the same state that we moved to.. We were assigned ***************** with sedgwick and ************************* with ***************** They have had our claim for 5 months now and are getting nowhere. Per our contract we are supposed to get reimbursed for what was damaged, broken or missing. We are getting no answers and they are going back and forth between Sedgwick and Intact. We have provided them everything we can. We are without beds and furniture and at this point would like some kind of resolution. Im an Amazon employee who was injured on the job on May 14, 2022. I was problem solving, on my knees bending over a case (box) of merchandise applying stickers to each individual item in the box when I was struck from behind by a u-boat carrying 72 empty totes. My lower back stopped the u-boat, prompting the u-boat pusher to walk around the stopped u-boat to see what he hit. He saw me on my knees with his u-boat against my back and said, sorry, sorry, sorry. He then went on his way. I reported it immediately and was escorted to Amcare by a manager. I have been out of work since that day more than 2 years ago, because Sedgwick, the third party company handling my case does not follow through with the doctors orders for treatment or medication. On January 9, 2023 I had a decompression laminectomy on L3 through S1. I was the first patient to be operated on that day but did not receive any medication, including pain medication, until numerous calls to Sedgwick and my attorney were placed. Sedgwick didnt approve any medication that day so, on my attorneys advice, I had to pay out of my pocket to get post operative medication, finally at 7:30 that night. Failed back surgery and 1 1/2 years later Im still waiting for treatment, still limping, still in agonizing pain, and still struggling with the fact that my nerve damage may be permanent due to lack of timely treatment. I had carpal tunnel release surgery, and I had surgery to remove scar tissue and relocate the nerve under the muscle. My original claim was extended due to my slow progress and complete numbness of my elbow and not able to left anything heavy such as a grocery bag. I saw my hand surgeon on 7/11/24. With original return to work date on the 18th the doctor wrote me a note stating that my return to work date would be August 1 and after that would be limited duty, no lifting more than 10 pounds until August 22. I had the doctor note my physical therapy schedule faxed over. I never heard from back examiner, which is nothing new. I constantly make phone calls daily and she never returns my calls or if she does she just leaves a voicemail and never tries to get a hold of me. I was told they need the physicians notes that was faxed over to them then they told me that they need more paperwork the original fax they sent incorrect, and I had to call back and get it re-corrected this time, they sent over a fax with the wrong provider on it and now my doctor wont sign off on it. Even the disability team at emerge ***** said that theyre giving me the runaround and its totally unfair since I pay for the service out of my Paycheck, we are going on August 1 with no resolution. I pay for short- and long-term disability out of my payment biweekly and when I need it I have to fight for it and its not right. Ive made daily calls since the 11th and Im truly tired. I have attempted to contact Sedgwick the insurance company in question for six I repeat 6 weeks. I have received zero response from the insurance company in question.The ********* comp case I was on with this insurance company the injury I endured which required three back procedures has forced me to apply for social security. Social security requires all payments made to me since the beginning of my ********* comp case. I have attempt to calling leaving messages when able to get through to an actual human being's voicemail I have attempted to chat I have attempted to email all to no avail no response. I am simply requesting a complete list of all payments made to me by ********* comp from the insurance company and question so I may complete my social security application I find it ludicrous that after nearly 2 months this insurance company has the inability to even return an email respond positively in a chat to direct me to an actual human being and or return an email correspondence in any way. I am extremely disappointed in the services rendered and I plan on following through if they continues to be this type of behavior with the better Business bureau as well as ************ legal services who has offered to file motions to demand payments made.I do not believe this is an unreasonable request I have attempts to contact my lawyer who no longer represents me he is also not returned my calls I even went through the links to attempt to contact the defense attorney for the company and question and she has not returned my calls so I am here injured three back surgeries attempting to acquire social security but unable to because the company and question refuses to send me my payment history. So I was injured on July 06 of 2024 working at Amazon on the 9th I visited the docter got all my paper work was unable to be accomendated then they referred me to a 3rd party workers comp provider Sedgwick and I started signing up and everything on the 8th I have heard nothing regarding my workers compensation claim I have tried using the communication center to see about updates mutiple messages no response back then I try the live chat they tell me to call so I call on hold for 30min then transfer and so on and so on for 3 hours no resolution said leave call back I get no call back its like the company ghosted me and not responding regarding my workers compensation claim I submitted documents the 14 and also the 9th doesnt show on the portal the over all communication isnt their and also lack of whats going on I have no idea of the issue thats occurring or the state of thing due to the difficulty of even getting a hold of someone. Ill I wish is to be contacted so I can get approved and overall finish the job and go be contacted
