Concentra in Saint Charles is meant for amazon workers during their workers comp cases. Their doctors classify cases as "non work" related even though they completely don't examine you!! The doctor did not touch me, give me X-rays, nor give me any type of pain relief. She immediately told my job it wasn't related which made me lose my case and return to work, just to be injured again.Business Response

Dear Sir or Madam:
I was seen at the Concentra clinic in *******, ** on Wednesday afternoon, 11/2/2022, for an employment physical and Tb Skin Test. I was told that I could have the Tb Skin Test read on Saturday at the clinic located at ********************************* XXXXX. However, I arrived at the location at 1pm and the location was closed. The 72 hour window expires today, 11/5/2022, at about 4pm.

I was told my full claim was denied but when I spoke with claims 2 body parts were supposed to be covered. Palpation never happened. My x-rays were mixed up. Wasn't asked to update medications. Provider did not wash hands nor wear gloves to do exam. Checked in 10:35 AM, waited in overcrowded lobby, no social distancing possible. Brought back to exam room 11:45 AM. Waited in exam room for provider until 12:50 PM. Told I had to see OM but they are at lunch until 2:00, so will have to continue waiting in exam room. Meanwhile there is a wedding shower going on in the nurses station, can hear everything in exam room and witnessed food & cake being eaten, gifts being opened when I was taken to the bathroom for a urine sample. Told to leave urine sample on nurses station counter, maybe they'll mistake it for champagne. Will follow up this initial complaint when I finally am released from this excuse for a health care facility.Initial Complaint

approximately around end of Jan or Feb. I had a follow up appointment in ******* *********** office when i was left sitting there for 2hrs with no explanation then was finally called in to see the doctor and when i asked her what was the reason that i was left sitting there she went ballistic jammed my leg back while i was laying on the table then put me back to full duty and told the employer i had nothing wrong i can return to work but i was seen at the san leandro location for concentra where the doctor took me back off work with crutches to walk on i ask the office manager to file a complaint and that never happened nor has she returned any of my phone calls this has resulted in loss of pay since then i have been fighting with the workers comp about the situationBusiness Response

From a medical perspective: Upon careful review of this case, it appears that the patient was seen initially by Dr. C**** on12/2/21 and diagnosed with Right knee strain. She was prescribed PT. On 12/30/21 she was seen again by Dr. C****, patient had not made any improvement thus same treatment was continued. She had not yet started PT. She then saw Dr. Wilson on 1/11/22 when again she was not better, she had increased knee pain with kneeling, squatting, thus MRI was ordered. She was maintained on modified duty all along. She was seen by Dr. Wilson again on 2/8/2022. Since she had been non compliant with PT after doing only 1 on 1/3/22, she was discharged from PT. On 2/8/22 Dr. Wilson noted "Symptoms are improving". Her right knee exam said: Appearance normal, complains of diffuse pain. Palpation normal. Full range of motion with pain. There was no mention of any discomfort with ROM testing during exam. Dr. Wilson discussed the MRI which showed "Low grade ACL sprain, mild distal quadriceps tendinosis, patellar tendinosis, proximal popliteus tendinosis, punctate Baker's cyst." She was given a copy of her MRI after it was discussed with her. Thus Dr. Wilson discharged her to Full duty with no further care. There is no mention of employer being notified. She then went to San Leandro and saw a locums provider on 2/22/22 who put her on modified duty of weight limit 30 lbs with no kneeling, squatting. There is no record of crutches given at that visit.
From a medical perspective it does not appear that there was anything that would corroborate the patient's claim of, "she went ballistic jammed my leg back"

I went in for PT on 09/29/2022 at Concentra in Conley Ga. The PT was working with another patient so she paid me very little attention when I came in. She had told me the day before to start using my crutches less and try to get down to one crutch I have an extreme amount of pain in my hip area and experience a lot of swelling when walking far. I have been compliant with using my crutches less when I'm home and I have something around me to catch myself in the event that I fall. The Physical Therapist(Renee Richardson) reported back to my employer on my documents that I wasn't compliant with HEP (Home Excercise Plan) without even asking me if I've been doing my Hep which I have. She didn't ask me anything when I came in she sent me straight to a exercise bike to get started while she worked with another patient . I'm sure they have camera's there and can go back and see we had no interaction aside from a simple greeting. She has no way of knowing what I'm doing at home if she doesn't ask. I was seeing a Orthopaedics specialist for two months prior to coming to concentra for PT. He ordered that I go on crutches for 4 weeks on 09/14/2022. The only reason I came to Concentra is because my other doctor isn't covered by WC. Concentra hasn't received my MRI to know if I have something deeper going on I'm swelling pretty bad everyday. She wanted me off the crutches my second session which was only a couple days after I started. She didn't fully evaluate me. I went in for my 3rd session which was one day after my 2nd session she wrote that I wasn't compliant because I experience a lot of pain with walking trying to get into the building from the parking lot so I used my crutches. My 1st visit with the PT was on 9/26/2022 she gave me 2 days to stop using the crutches despite the specialist orders. It is not legal and completely irresponsible to lie on medical documents. I'm having a really hard time and this is added stress. I will be contacting the state today.Business Response

I reviewed the case and did some further investigation on this. Please see a brief synopsis/highlights of the case.
Patient was seen at Emergency Room before starting treatment with Concentra. Referred to an Ortho that she saw twice before they realized it was under workmen's comp, and she came to Concentra.
Patient was never given crutches by Concentra. In the transcripts by Dr. Montaque it states, her Ortho told her to start using the crutches again since she was struggling to get back in the office.
When she was first seen on 9/22/22, pt had therapy referral but didn't start Physical Therapy until 9/26/22 after her follow up visit with clinician.
Ms. ***** ***** was seen for a Same Day initial Physical Therapy Evaluation on 9/26/22 to treat diagnosis of Right Hip Contusion. Therapy orders were written for three times a week for 2 weeks.
Patient was brought over by staff with crutches and was non weight bearing on Right lower extremity.
Therapy evaluation was completed and patient was issued a Home Exercise Program. Physical Therapy expectations were reviewed as well.
Patient was instructed to try toe touch gait with crutches in preparation for decreased dependence on assistive device.
On subsequent two visits, HEP was progressed and as part of treatment, patient was educated regarding weaning from crutches secondary to x-ray of hip negative and no known contraindications.
Patient was instructed towards the end of the next two sessions in and practiced gait with one crutch with standby assistance by Physical Therapy.
Ms. ***** was instructed to practice at home in close quarters, but to continue use of both crutches for now for safety.
Physical Therapy noted no observable contraindications in medical note regarding any reason why normal progression of gait would not be indicated as part of treatment plan.
Patient did complete 3 visits of Physical Therapy and has not shown up for the last 3 visits. The staff has called her and left messages. Per the transcriptions, patient is currently not working due to no light duty at work.
The last two notes by Renee Richardson, the 2nd visit Renee instructed the patient to try to do Home Exercise Program with only one crutch with safety around her and to use both crutches when she outside. The 3rd note, it does say "non-compliant" with Home Exercise Program, but doesn't go into details from what I read.Initial Complaint

I called Concentra and was told I could go and pick up a copy of my TB test I had received there at the beginning of August. I went to concentra today and stood in the line and waited. And another man came into the line and cut infront of me. They serviced this man and complained to me that I wasn't standing where I should have been. When in reality the man cut in front of me and the line shifted because other people wanted to be seen so they would know he cut the line. When I respectfully brought it to staff attention that he cut in front of me. She didn't address the man she instead had me stare my name out loud which is in appropriate as that man could have had malice towards me because I addressed the fact that he cut the line and now he knows my full name. In addiction to that she told me he was allowed to cut the line because he had been seen already and was just here to get his paper work. I let her know- I have been seen already and I'm just here to get my paperwork. And further more there are no signs up to separate the lines. She told me the signs were outside the door. That makes absolutely no sense. This company has access to my medical records and have made me obtaining a copy (which is my right) difficult but also they use favoritism and make up rules and say things in a way as to demean people instead of creating a fair and equal environment for which people can be serviced.Initial Complaint

I was injured at work and doctor umphreys who is an amazing doctor was the only doctor to help me then my job decided to contact the director doctor hudwig I believe who changed my entire work restrictions (who has never seen or met with me ever) and has now made everything worse this has created trauma and other issues I am also going to the media and medical examiners board they only changed my work restrictions because my job asked them to they work for the job not the patientBusiness Response

I do not have a head/cuncussion specialist at all. So Dr. Umpries was the only doctor who knew about my anxiety ptsd cuncssuoon symptoms. No doctor who has never seen me or spoke to me has the right to change work restrictions without even speaking ti me . I'm worse now then before. There were a reason those restrictions are in place. Yes this is in reference to a Hearing and Vision test I took at Concentra Medical Center on 08/5/22 for my job. The thing that concerns me is how the doctor at Concentra tried to tell me my hearing in my right ear was very bad. And even on the paperwork it said both my left and right ear was bad. So I was concerned about it. So I went to another hearing doctor 18 dyas later for a second opinion. This was at Consulting Audiology Association. The doctor told me my hearing was okay. So I dont know what Concentra problem was with me. For also they had me take the hearing test twice. I believe it was a form of harrassment. I found this very disgusting. So below I have copies of the tests from both companies. And I would like to thank you for your time.
