    I went to a clinic that promised to treat a personal condition for me and they closed down shop halfway through the treatment plan. My loan was through ***** lending and the loan was for $5000. I now owe $7,186.25! The clinic was named Crestside Medical Clinic, 4330 Medical Drive, suite 150. Phone Number: ************. I really need help with this. Thanks in advance for your, effort, and consideration.

    In December 2022, I went to this place for a "free" consultation and never ended up going back after the free period. Then about 1 1/2 years later, I get a letter from ***** Services showing that I owe a loan to them for $5000 plus interest that all goes back to Crestside Medical Clinic. I have called the *********** that gave Crestside the money for the loan and they have no way of finding it without an account number and obviously, I do not have that. The loan that they took out in my name will start needing to be paid in January 2025. So I need help on this asap.

    Paid for treatment and follow-up care in full. Company closed up and disconnected the phones. They will not honor their agreement or complete treatment or provide any follow up care as noted in the agreement attached:Procedures promised but not provided:1) Six (6) Acoustic Pressure Wave Treatments 2) Includes one (1) additional consultation and up to two (2) additional cycles of the six (6) treatments 3) ********** services may include up to two (2) tune-up sessions yearly as needed, medications and other services as deemed necessary by Clinic Doctors or staff

    Purchased acoustic Wave therapy for *** Received 4 treatments. Came in for my appointment and the doors were locked. Have not been able to make contact with anyone.
