      This provider refuses to provide me with an itemized bill including CPT codes. It is my right as a patient to receive the itemized breakdown of services in order to verify that the charges are correct. I have requested the itemized bill, including CPT codes, 7 different times. I have requested them verbally on multiple phone calls, on a voicemail I left for the customer service manager, in writing that was mailed to them using their pre-addressed envelope, and on a conference call with a UHC representative. They refuse to provide the codes so I have not been able to verify my bill is correct. On the date of service, the provider struggled to find the proper epidural size kit and had to replace my epidural with a second one. The nurse who witnessed this advised I confirm the bill before paying to ensure I was not double-charged. Millcreek Anesthesia has a local reputation of improperly billing and overbilling patients. They do not have an office phone number or office I can visit in person--it all goes directly to their billing, which they have outsourced to another company overseas. Their address is a P.O. box. It is not possible to call and speak to an accountant or provider at Millcreek Anesthesia.
      They could be deliberately overcharging patients and there is no recourse. There is no one who can answer questions or assist customers with inaccurate billing.

      I had a C-section performed on April 7, 2022. The bill I received from the anesthesiologist was much higher than I expected in the amount of $1065.25 (acct #*********). The anesthesia services were also the worst I have ever experienced during 6 surgeries. I tried talking to Billing about it and they said to send an email that they would forward to the anesthesiologist. I did and didn't hear anything back so I called to check on the status. I have called around a total of 7 times. I was told it was still under review and would receive a bill once it was complete. The next statement I received was from a debt collector. I called Billing and they would not tell me whether or not the anesthesiologist ever received my email & the only contact that is listed on the company website is the billing info. Here is part of the email I sent on July 25th:
      "On April 7th I had a C-section. My OBGYN and the hospital had both quoted me an estimate prior to the surgery. I did not get a quote or an estimate from an anesthesiologist and to my knowledge I did not sign anything stating I was aware of the upcoming charges.By far the most difficult part of the surgery were the anesthesia services. I experienced extreme swings with the anesthesia that I do not recall being part of my experience with my first C-section. I was also disappointed that a medication that I knew had made me throw up (duramorph) with my first C-section would still be used despite me mentioning my preference not to use it. I was hoping another option would be available, but I was not given that choice. I was very surprised when I got my bill to find that the anesthesiologist charges exceeded those of my OBGYN- which included some office visits, an ultrasound and the surgery. The anesthesia bill is much higher than what I was charged for my first C-section. Lastly, there is a $210 charge for Pain Management Service the day after my surgery that I couldn’t account for. "
