      I am being billed for a test I was never billed for before due to getting a new doctor at same clinic. Previous doc had always billed my yearly tests as routine. New doctor put a code in that does not pay for my Vitamin D test, even though it was paid through all the years. When I asked my doc about, she put in writing that she would change it to screening, but, a manager jumped in and told me I couldn't talk to doc about this anymore. I asked where to appeal, and sent all info to email address given which did not come back undeliverable. After some time past, I contacted ****** under MY Chart again and was then contacted by ******** who called to say she never received my info and their denial of service stands. WITHOUT MY ADDITIONAL INFO?!! She tells me a letter was sent telling me so. No. No letter. So after reading the emailed letter from ********, the board admits they made a mistake all previous years, but now I have to pay, because I am prescribed Vitamin D. I certainly would have told them not to test, as buying vitamin D is way cheaper than paying for this test I was never billed for before. I sent a certified letter to the appeals board, but they are ignoring me now. I should have been warned about this SURPRISE bill. I want this bill rescinded.

      I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear belo

      I still have a right to have all questions answered in writing.  As you can clearly see, the doctor said she would change it to screening so I would not be charged.  You reviewed without this information.  Please explain your position with the extra added information, like the doctor stating she would change the coding.  You know very well my information was swept under the rug even though I submitted in a timely manner to ******************************  Please answer my questions submitted so I may further understand why the doctor stated it would be changed to screening and she did not.  And you sure can change it.  Worked in the filed long enough to know.  You admitted to your incorrect coding, now please take responsibility and change the coding so this claim will now be paid in full.  Thank you.

      On 5/9/2022 Provider asked to do US Pelvis Complete Non OB having CPT code 76856.On 5/13/2022, I called Pro healthcare on ************ to get estimates for above US (Ultrasound). ********* from Pro healthcare suggested to get this US done at their clinic location located at *****************, Waukesha ** estimates total out of pocket costs will be $253.55 since performing this same US at the Hospital location is higher.I scheduled this US test for 6/14/2022 at the above clinic location and made a payment of $253.55 through phone on this same day while I was talking with billing department at the time of the test. Billing Department had the physicians order in their system directly faxed at the time of the test I was told all bill payment has been made for this order and there is no outstanding balance left or due on this account.In July 2022 Pro health care sent to first bill with statement date 6/15/2022 to my home address by mail I still have to pay outstanding balance of $348.70 with the location different than where this test was done. I called several times after this to the billing department why is this extra costs? I also told them on phone this test was done at the clinic location and not at hospital location. I was told they will call me back after discussing with their coding department, every week I was calling and I was getting the same answer they are waiting for the review to happen with coding department. I also told them they to leave voicemail in case I did not answer when they called, no one called me regarding my disputed bill. I asked billing department to provide me CPT codes for which tests were done, they stated me every time they do not have those CPT codes nor EOB (Explanation of Benefits).I finally paid outstanding balance of $348.70 on 7/25/2022 after I received 2nd bill with statement date of 7/17/2022 since I did not want my outstanding balance to be reported to Pro healthcare collection department.

      I have not been feeling well for about 4 or 5 years. I've been to many different doctors and they could not figure it out. About 3 years ago I bought a pulse oximeter and saw my heart would race and my oxygen would go down. I finally had heart tests and was put on medication for the racing heart. I didn't improve.About 4 months ago I landed in the hospital due to pulmonary edema. They said it was because I m fat! When I came home the heart doctor's ****** called and asked me to come in. I was told I was in stage 3 diastolic heart failure and had been for many years. She could not answer why nobody told me or bothered to put me on the medication I needed. I can't get back the years they ignored me, but I want to know why and when this started.
