I went to Coast Dental on May 14th to establish dental care with a new dentist as I have recently relocated to this area. They performed x-rays and periodontal maintenance cleaning, which totaled $1667 (me paying $675.80 and the remaining balance was to be submitted to insurance) per the treatment plan they provided me. They in turn submitted a claim to my insurance company for over $4k which of course was denied and now they are trying to bill me for an additional $797. There is no way that anyone is paying almost $1500 for a deep gum scaling/cleaning. I believe they are purposefully submitting fraudulent claims to the insurance company for services, based off this transaction and the *** (explanation of benefits) I received from my insurance company for this service.Business Response

Coast Dental Riverview Date of service : 06/10/2025 Amount if money: $308 (second opinion new dentist) and $451 for quote cavity filling from new dentist Business gave unnecessary diagnosis of periodontal disease that needed deep teeth cleaning quoted at $1080 and 1 cavity ($59). Got a second opinion on 6/11/25 with out-of-network provider from insurance did not have periodontal disease, and found 2 cavities just needed prophy cleaning, which they charged $308 - out of network and quoted $451 for 2 cavities. Tried contacting the office manager and completely dismissed my call, did not provide an email. Contacted corporate office, left message and says they will call in 7-10 business days. Have not received a response. Sent email today to a corporate email I found online and to the office email provided by customer service. Have not received any response.Business Response

Ms. Lemois, I do not see where you paid Coast Dental $1,000.00 for services. You were seen on 6/10/2025 for a complete exam and x-rays, you were provided an ESTIMATE for recommended services based our providers diagnosis and what he felt was the best course of treatment. I understand you went for a second opinion to another Dental office, where they recommended a different option to you and that is your right to do. However, that doesn't mean that our provider over diagnosed, that is that other Dental Office opinion which is irrelevant as we can opinionate they under diagnosed you. We regret you did not feel comfortable with our provider, but we are glad you were able to resolve your Dental needs elsewhere. However we do not feel any type of refund for services you did not receive or pay us for is warranted.

I went to Coast Dental for my first routine cleaning with this office on 8/21/24. At that time, they completed x-rays and determined I needed a deep cleaning. They reported they had a breakdown of the percentage of what my dental insurance would cover and I spent approximately $400 out of pocket on that date for services. It was the technician's first day working and it took approximately 45 minutes to complete my x rays. When the dentist came in to review them, he identified they had been completed incorrectly and were not readable as they were upside down and backwards. New x rays were not completed. I subsequently received a bill for $547. My dental insurance, ******, denied payment for x rays as well as the deep cleaning because that is apparently not covered. I have attempted to contact Coast to identify my concerns and resolve this matter on numerous occasions and have left an estimated ***** phone messages at numerous phone numbers provided by Coast and have never received a call back. Coast continues to send me twice weekly text and email bills and I am concerned they will send me to a collections agency and impact my credit. The office where I was seen was subsequently closed however other offices of Coast remain open. The x ray service they are charging me for was unreadable and they further violated the No Surprises Act by not adequately explaining additional fees at the time of service if insurance denied the claim.Business Response

Dear Ms. Register, We do apologize that your insurance did not cover your services. Please understand that treatment plans signed by patients are an estimation of coverage not a guarantee of payment. Ultimately patients are financially responsible for services their insurance plan denies. I have attached your records for clarification 1.The X-rays taken are legible to read and diagnose.There are only three that are flipped, but that doesn't impede the doctor from being able to diagnose. The software we use allows us to flip and rotate x-rays. 2. We are in Network with Humana and the fees on the treatment plan and your denied services align with IN NETWORK allowed, therefore your balance and responsibility to pay does not meet the "NO SURPRISE ACT" as that clearly is for patients being billed UNKNOWINGLY OUT OF NETWORK FEES. 3. I also have attached the EOB for your view. The plan denial of your services for (age limitations) as highlighted is not common nor logical for the services rendered, and we encourage you to reach out to them and appeal/question their decision. We have never received a denial for such limitation and we believe it may be an error on Humana's part. We will not be writing off your balance as it is your financial responsibility and legally we are able to continue communications in order to collect on a bill until paid in full.

I am rejecting this response because:It is not an acceptable business practice to message a patient twice weekly with automated messages for months on end without responding to a single email or phone call. It should not take involvement from the BBB to respond to a client complaint. You claim your services are within the allowable range, however it is outside of industry standard and price to charge a patient over $900 for a deep cleaning and standard x-rays. I am requesting a reduction in fee at the least.

On May 14, 2025 I had my first initial appointment with Coast Dental as a new patient. I was instructed by the staff that this was my initial visit and would be required to make a co payment as my insurance company was ******* and this was the agreed amount. I had my appointment the office did x-rays and I was given a brief consultation. I told them I would schedule an appointment after I had my schedule from work. On 5/30/2025 I received an email saying I owed another $58.00. After calling Coast Dental on 6/03/2025 I was told that some of the x-rays were not covered and I owe $58.00. My statement is that I did not know or authorize any additional services other than my initial visit and co-payment. I am rejecting this response because:I had not requested any additional services except an initial consultation. As for Coast Dental “estimate” claim, they are in the business of knowing what is covered for first visit, that is what they do. They know what each provider covers and what they don’t. To say it is up to the patient to interpret coverage is ridiculous. Coast Dental has the total responsibility of knowing what the insurance providers cover. To claim ignorance on their part is not acceptable. My insurance company stated that the dental office knows EXACTLY what is covered for all services, especially an initial consultation. If Coast Dental charged $58 too much, that error (if that’s what it was) is totally on them. My final point is that it is their responsibility to know what is covered and what is not. Thank you.

Mr.Kowzan, This issue has nothing to do with our knowledge of coverage. It has to do with your Insurance carrier applying your deductible to that date of service. We apologize for your confusion in understanding your plans limitations and deductibles. Filing a claim on your behalf is a courtesy, patient are ultimately responsible for any services not covered or in this case deductibles that an insurance implements. All patients are provided an estimated treatment plan prior to services being rendered to which they have to opportunity to review prior to signing. We did not provide you an UNCOVERED SERVICE, once again your plan applied your due deductible to that date of service. There is nothing more we can do or say to remedy your complaint. This complaint stems from the Coast Dental *************************************************************** Location .*************)The issue at hand is that coast dental CONTINUES to text me over 14 times that I owe them $64.00 for my regular cleanings.(Note : on 5-29-25 the text states that I NOW OWE 128.00)I never get a straight answer from this office if and when they respond to This fraudulent billing activity by their office Cigna ,my dental provider informed them via phone and fax multiple times, that I dont owe anything for regular dental maintenance.Cigna also stated it appears coast missed the 15 day deadline to file a Claim for reimbursement so they are passing it on to me which is fraud !!!Ive finally had enough and filed a complaint with Cigna and now the BBB in regards to this matter.I also asked the representative (******) ,on 5-19-25 that I only want my records and my two childrens records and dont want to do business together anymore.She informed me that I have to Fill a form out and email her back which I did on 5-20/25.On 5/21/25 via email she stated the request is received please allow ***** hours for release of records to my email ********** of 6-1-25 no response/records received and they keep ignoring my multiple requests for answers along with rescheduling on their on appointments for myself when I stated . In writing that I will NOT be there anymore *********** has multiple fraudulent issues it practices from billing , to not sending records to Customers when requested and making appointments for customers without their approval and or knowledge Even after the customer told Them to stop!!!!Last. Coast Dental is balance billing patients. It is my understanding that is illegal. I was shocked at the amounts expected from Coast Dental in my estimates and even more shocked when I found that the estimates were the difference to the amounts they charged and the amounts my insurance company allowed as reimbursement (balance billing). The $287 is disputed because I was given a good faith estimate of $0 which is what my insurance said I owed. Coast Dental then billed me $287 and has harassed me weekly about my "balance due". I spoke with the office who said that I signed a document stating that I would pay for what was not covered. I am requesting a copy of the document. I have never been balance billed by any healthcare professional after an insurance payment. I should only owe my co-pay or what I use beyond my annual allowance for which I still have $2,000 left. I went to Coast Dental on ***************** because they were assigned to me by my ******** Provider. I was given an ESTIMATE on costs for 2 crowns and a cleaning. The cost was $3,000. After seating the crowns, Dr. ****** only charged me $2,000. Coast Dental then started DEMANDING I pay the additional $1,000 and even threatened legal action against me. Dr. ****** told me he saved me some money and only charged me the $2,000. He left the practice shortly after my appointment. The dentist was extremely rough and yelled at me that I bit him! Mr. ******* though we do sincerely regret your feelings of frustration with not being able to locate an Oral Surgeon. We would like to clarify the office is in no way obligated to find an Oral Surgeon that takes your insurance. The office properly provided you with a Referral to which you in turn could have contacted your insurance plan for a more detailed option of providers available to you. You are very adamant that this was the offices responsibility to provide you and have made no efforts to reach out to your insurance for assistance. Our staff has in no way mistreated or disrespected you. Those are false claims on your part and very disappointing. I am rejecting this response because: the business was acting kind towards me in response only because it had to go through BBB first they act so accusatory & toxic with me on phone as I have mental illness Obsessive compulsive disorder & secondly occasionally other ill styles all cruxed at a disability "condition" autism spectrum disorder there is great evidence that the coast dental ***** personal the has there own mental illness I have only one evident directly from them in which proves unfair & conflicting reasons to not refund me and unfair blame from them in which an email from ***** Finnaginn there attorney who said in an email she would refund me & in final email refused in a manner involving unfair & irrelevant blaming towards me they provided me with one referral the day of service & it is a practice somehow that they are connected to & is by some means more solid (respectful) & by some means less solid (disrespectful) things folded between them & me there policy's & how my mental condition clashed I never asked that they contact my insurance I asked that they provide me with additional referrals for necarcarey work but never said contact my insurance that was a office staff "********" idea it did seem far fetched but I believed her as they complained I tried to I hold them to that I also said with the urgency in this dental work which still is hasn't been done coupled with my week points I was asking as a favor to get me some names orel surgeons I tried being diplomatic just like that thay told me they promised to provide me with names from my insurance at some point & would try to do so with In 2 weeks but couldn't promise I waisted 15 waiting then they became outrageously insulting disrespectful ( see accusations about me)& things between myself and them escalated they are acting kind to me in response only because BBB is seeing* once 1. I have tried calling several times and have been told just to keep my phone available and someone will call me. I've been waiting weeks. I'm being charged $203 for a visit my insurance covered. They told me that all I owed was for the cancer screening. I waited at the office for over 5 hours that day! Then I got told to download affirm. I got fed up for being there for so long that I did. I did it to cover a procedure that they told me I had to come back in 2 weeks because they had "ran out of time" to do my cleaning and such. This was on March 28th. I went back up there that Monday (March 31st) and cancelled the appointment and to which I didn't get a notification that they cancelled it until April 8th, the days of my appt. Affirm is trying to charge me for the procedure that I NEVER got done as well. This is for $428. I went and told the Lake City office and was told that they "didn't know how" to cancel it. I opened up a dispute with affirm and they are trying to get ahold of the coast dental in ********* but they shut down. I was given an email for the coast dental in ********* by an employee and she told me to send her screenshots, in which I did before they shut down and no one has responded. In June 2024, I went in for a routine cleaning and clear aligned consultation. I was sold an electric toothbrush that stopped working within a week. I returned the toothbrush and was told that I would be given a refund within 2 weeks.
